## Narrative

As the FAA filed it, in capitals; the FAA replaces private details with ^PRIVACY DATA OMITTED^.

(-23) AIRCRAFT #455YV (DASH-8) WAS PUSHING TAIL NORTH OUT OF TERMINAL 4 GATE B-1C. THE PUSHBACK AGENT DID NOT CLEAR THE 30FT TURN LINE AS HE TURNED THE TAIL SOUTH TO LINE UP ON THE PAPA LINE FOR THE AIRCRAFT TO TAXI OUT. THE PUSHBACK AGENT PUSHED THE TAIL INTO AIRCRAFT N17358 (CRJ-200) THAT WAS PARKED TAIL FACING SOUTH IN GATE B-18B. THE ELEVATOR OF THE RJ SLICED INTO THE RUDDER OF THE DASH-8. WING WALKER ON THE FIRST OFFICER'S SIDE BROKE AWAY TO GO SUPERVISE ANOTHER FLIGHT. AS THE DASH-8 WAS TURNING TO BE LINED UP WITH THE PAPA LINE THE CAPTAIN'S WING WALKER NOTICED THAT THE TAIL OF THE DASH-8 WAS APPROACHING THE TAIL OF THE CRJ. HE THEN THREW UP HIS WANDS INTO THE STOP POSITION BUT THE TUG DRIVER WAS LOOKING TO THE NOSE WHEEL AND DID NOT RESPOND IN TIME. THE DASH-8 RUDDER TAB WAS SPLIT BETWEEN STIFFING MEMBERS AND THE SPLIT STOPPED JUST BEFORE A SPAR TYPE NUMBER. TORN ALUMINUM FROM THE DASH-8 RUDDER TAB SCRAPPED THE ALUMINUM SURFACE OF THE CRJ ELEVATOR. THE CRJ ELEVATOR NEED MAJOR REPAIR AND THE DASH-8 ELEVATOR TAB NEEDED TO BE REPLACED.

## The NTSB's investigation of this event (1)

From the NTSB's public API. Aircraft are matched to registry aircraft by N-number and event date, checked against the reported serial number, make and model; the registry's history has gaps, so older matches can be wrong. Compare the reported make and model.

- [NTSB LAX03LA295](https://openairdocs.org/ntsb/LAX03LA295), Sep 21, 2003: de Havilland DH-8-202 (N455YV), Phoenix, AZ, substantial damage. Probable cause: the ground agent's inadequate visual lookout during push back, which resulted in his failure to maintain clearance between the airplanes.

From the FAA Accident/Incident Data System. Reports are matched to registry aircraft by N-number and event date, checked against the reported make and model; the registry's history has gaps, so older matches can be wrong. Compare the reported make and model. For accidents, the NTSB's own investigation is the fuller record.
