## Narrative

As the FAA filed it, in capitals; the FAA replaces private details with ^PRIVACY DATA OMITTED^.

(-23) ON SEP 25, 2003, AT APPROXIMATELY 0651 AIRCRAFT PERFORMED UNEVENTFUL POWER BACK FROM GATE C-17 AT DFW AIRPORT. UPON INITIATING TAXI, SMOKE AND ACRID FUMES WERE REPORTED IN THE CABIN BY CREW MEMBERS. AFTER A SECOND REPORT, WHEN CONDITIONS WERE REPORTED WORSE THE EMERGENCY EVACUATION PROCEDURES WERE INITIATED. L1, L2, R1 DOOR SLIDES WERE SUCCESSFULLY DEPLOYED. THE TAILCONE JETTISONED, BUT THE SLIDE DID NOT INFLATE. 14 FIRST CLASS, 73 COACH PASSENGERS AND CREW MEMBERS EVACUATED WITH (8) EIGHT MINOR PASSENGER INJURIES.

## The NTSB's investigation of this event (1)

From the NTSB's public API. Aircraft are matched to registry aircraft by N-number and event date, checked against the reported serial number, make and model; the registry's history has gaps, so older matches can be wrong. Compare the reported make and model.

- [NTSB DCA03IA058](https://openairdocs.org/ntsb/DCA03IA058), Sep 25, 2003: McDonnell Douglas MD-80 (N462AA), Dfw, TX, incident, no damage. Probable cause: The improper maintenance of the tailcone evacuation system, which prevented the evacuation slide from inflating during an evacuation due to smoke/fumes in the cabin.
