## Narrative

As the FAA filed it, in capitals; the FAA replaces private details with ^PRIVACY DATA OMITTED^.

(-23) ^PRIVACY DATA OMITTED^ WAS A "D" LICENSED PARACHUTIST, USPA #37837 EXP. 2-29-04, LICENSE #D-13328, WITH 1700 HOURS AND 20 YEARS EXPERIENCE. ^PRIVACY DATA^ WAS FATALLY INJURED ON SEPTEMBER 26, 2003. ^PRIVACY DATA^ DID OT HAVE ANY PARACHUTE CLUB AFFILIATION. THE MAIN CANOPY EQUIPMENT IS A PERFORMANCE DESIGNS SABRE 120, #LS120-015817, DATE OF MFG. JUNE 1998. THE HARNESS/CONTAINER IS A SUN PATH JAVELIN MODEI TJN, DATE MFD. JUNE 1998, S/N 15059. THE DATE OF THE LAST REPACK AS LISTED ON THE REPACKING DATA CARD WAS 8/23/03 AND THE SYMBOL SEAL WAS QR6. FURTHER EQUIPMENT INVESTIGATION WERE ACCOMPLISHED BY A LOCAL MASTER RIGGER, ^PRIVACY DAT^ USPA D-4437. HIS REPORT INDICATES THAT THE RIGHT SIDE LOWER CONTROL LINE WAS KNOTTED AROUND THE TAB OF THE SLINK ON THE RIGHT RISER. THIS WOULD EXPLAIN THE CANOPY SPINNING TO THE RIGHT AS DESCRIBED BY WITNESS REPORTS. WHEN THE DECEASED RELEASED THE DEPLOYMENT BRAKES THIS LINE LOOPED AROUND THE SLINK TAB AND CAUSED THE CONTROL LINE TO HANG UP AND THIS CAUSED THE CANOPY TO BEGIN SPINNING TO THE RIGHT. ^PRIVACY DA^ ALSO INDICATED THAT THE RESERVE RIPCORD PIN MOVED SMOOTHLY IN THE RESERVE CLOSING LOOP AND THAT THE RIPCORD HOUSING WAS CLEAR OF ANY OB STRUCTIONS. HE ALSO TESTED THE CUTAWAY SYSTEM AND FOUND IT OPERATED SMOOTHLY.

From the FAA Accident/Incident Data System. Reports are matched to registry aircraft by N-number and event date, checked against the reported make and model; the registry's history has gaps, so older matches can be wrong. Compare the reported make and model. For accidents, the NTSB's own investigation is the fuller record.
