## Narrative

As the FAA filed it, in capitals; the FAA replaces private details with ^PRIVACY DATA OMITTED^.

(-23) ON DECEMBER 15, 2003, MR. INUI KOKI DEPARTED SAN BERNARDINO INTERNATIONAL AIRPORT (SBD) SAN BERNARDINO CA. EN ROUTE TO FRENCH VALLEY AIRPORT, TEMECULA, CA. ON HIS RETURN FLIGHT IS WHEN THE ACCIDENT OCCURRED. WITNESSES REPORTED THAT THE HELICOPTER WAS FLYING ABOUT 2 MILES SOUTH OF SBD WHEN THEY HEARD THE ENGINE START TO MISFIRE. THE HELICOPTER TURNED TO THE WEST AND APPEARED TO BE ATTEMPTING TO AUTOROTATE TO THE STREET BELOW. ABOUT 200 TO 300 FEET ABOVE GROUND LEVEL (AGL), THEY OBSERVED THE MAIN ROTOR BLADES COMING UP, AND THEN THE BLADE'S ROTATION STOPPED COMPLETELY. THE HELICOPTER FELL STRAIGHT DOWN COMING TO REST ON THE CITY STREET BELOW. THE HELICOPTER RECEIVED MAJOR DAMAGE, AND MR. KOKI RECEIVED FATAL INJURIES UPON IMPACT.

## The NTSB's investigation of this event (1)

From the NTSB's public API. Aircraft are matched to registry aircraft by N-number and event date, checked against the reported serial number, make and model; the registry's history has gaps, so older matches can be wrong. Compare the reported make and model.

Probable cause: a loss of engine power due to the pilot inadvertently turning off the magnetos and his failure to initiate an autorotation and to maintain main rotor rpm. A factor in the accident was the pilot's use…

From the FAA Accident/Incident Data System. Reports are matched to registry aircraft by N-number and event date, checked against the reported make and model; the registry's history has gaps, so older matches can be wrong.
