## Narrative

As the FAA filed it, in capitals; the FAA replaces private details with ^PRIVACY DATA OMITTED^.

(-23) DURING A TAKE-OFF FROM PAGE, AZ (PGA) AND AS WITNESSED BY ANOTHER PILOT, THE PILOT (PLUS 2 PASSENGERS) ENCOUNTERED A STALL AFTER LEAVING GROUND EFFECT @ THE 3000 FT. MARKER OF RUNWAY #15. THE A/C THEN SETTLED BACK DOWN INTO GROUND EFFECT BUT CONTINUED TO ATTEMPT TO GAIN ALTITUDE FOR ANOTHER 1000 FT. (APPROX) OF RUNWAY WHICH RESULTED IN A SECOND STALL, SETTLING AGAIN INTO GROUND EFFECT. THE THIRD AND FINAL ATTEMPT TO GAIN ALTITUDE RESULTED IN YET ANOTHER STALL WHICH WAS MORE PRONOUNCED CAUSING THE A/C TO CONTACT THE GROUND, NOSE DOWN, L/H WING LOW. DAMAGE TO THE A/C RESULTED IN A FRACTURED NOSE WHEEL FORK AND BUCKLING (DOWNWARD) OF THE L/H WING APPROX. MID-SPAN. THE PROPELLER ALSO CONTACTED THE GROUND. THERE WERE NO (REPORTED) INJURIES TO THE THREE (3) OCCUPANTS AND NO POST-CRASH FIRE.

## The NTSB's investigation of this event (1)

From the NTSB's public API. Aircraft are matched to registry aircraft by N-number and event date, checked against the reported serial number, make and model; the registry's history has gaps, so older matches can be wrong. Compare the reported make and model.

- [NTSB LAX04LA191](https://openairdocs.org/ntsb/LAX04LA191), Apr 8, 2004: North American Aviation Div. Navion (N91729), Page, AZ, substantial damage. Probable cause: the pilot's failure to attain and maintain a sufficient airspeed, which led to an inadvertent stall mush in high density altitude conditions.

From the FAA Accident/Incident Data System. Reports are matched to registry aircraft by N-number and event date, checked against the reported make and model; the registry's history has gaps, so older matches can be wrong. Compare the reported make and model. For accidents, the NTSB's own investigation is the fuller record.
