## Narrative

As the FAA filed it, in capitals; the FAA replaces private details with ^PRIVACY DATA OMITTED^.

(-23) AIRCRAFT DEPARTED LAKE PILLSBURY AIRPORT (IQ5) DIRECT TO SANTA ROSA (STS). PILOT OBSERVED THAT AIRCRAFT MAIN LANDING GEAR HAD FAILED TO EXTEND DURING FINAL APPROACH TO STS. PILOT CYCLED GEAR NUMEROUS TIMES AND NOTED THAT A SAFE GEAR INDICATION WAS NOT ILLUMINATED AND THE GEAR MOTOR WAS CONTINUOUSLY RUNNING. PILOT ATTEMPTED NUMEROUS EMERGENCY GEAR EXTENSIONS THAT ALSO FAILED. PILOT PERFORMED A VISUAL INSPECTION OF THE HYDRAULIC RESERVOIR AND NOTED A DEPLETION OF FLUID WITH EACH GEAR EXTENSION/RETRACTION ATTEMPT. THE PILOT WAS UNABLE TO RETRACT THE NOSE GEAR. THE AIRCRAFT LANDED WITH NOSE GEAR EXTENDED AND NO MAIN WHEELS. UPON TOUCH DOWN THE PILOT LOST CONTROL OF THE AIRCRAFT, AND THE AIRCRAFT DEPARTED THE RUNWAY STRIKING AN AIRPORT TAXI SIGN. THE AIRCRAFT SUSTAINED SUBSTANTIAL DAMAGE.

## The NTSB's investigation of this event (1)

From the NTSB's public API. Aircraft are matched to registry aircraft by N-number and event date, checked against the reported serial number, make and model; the registry's history has gaps, so older matches can be wrong. Compare the reported make and model.

- [NTSB LAX04LA230](https://openairdocs.org/ntsb/LAX04LA230), May 30, 2004: Cessna T210M (N121JR), Santa Rosa, CA, substantial damage. Probable cause: Maintenance personnel's improper installation and servicing of a retaining ring, which resulted in the failure of the landing gear door actuator and failure of the landing gear extension system…

## Other reports on N121JR (1)

Matched to the same aircraft by N-number and date.

- [Incident 19791029053629I](https://openairdocs.org/incident/19791029053629I), Oct 29, 1979: Cessna T210M (N121JR), Santa Ana, CA, Minor damage. Pilot landed fast due to crosswind, unable to stop on the runway.

From the FAA Accident/Incident Data System. Reports are matched to registry aircraft by N-number and event date, checked against the reported make and model; the registry's history has gaps, so older matches can be wrong. Compare the reported make and model. For accidents, the NTSB's own investigation is the fuller record.
