(-23) ON-SCENE INVESTIGATION REVEALED THAT THE AIRCRAFT WAS INVOLVED IN A STALL/SPIN AND WAS DESTROYED BY IMPACT AND POST-IMPACT FIRE AFTER COLLIDING WITH TUNDRA-COVERED TERRAIN. THE AIRPLANE WRECKAGE WAS OBSERVED IN A NEAR VERTICAL, NOSE DOWN ATTITUDE, IN THE SOFT TUNDRA-COVERED TERRAIN. BOTH WINGS HAD SIGNIFICANT SPANWISE LEADING EDGE CRUSHING. THE ENGINE ASSEMBLY WAS BURIED IN THE TUNDRA. BOTH OCCUPANTS RECEIVED FATAL INJURIES. UNDER BLOCK 31 PILOT INFORMATION FOR REGULATORY CHECK RIDE IS NOT AVAILABLE.

## The NTSB's investigation of this event (1)

From the NTSB's public API. Aircraft are matched to registry aircraft by N-number and event date, checked against the reported serial number, make and model; the registry's history has gaps, so older matches can be wrong. Compare the reported make and model.

- [NTSB ANC04FA102](https://openairdocs.org/ntsb/ANC04FA102), Sep 4, 2004: Piper PA-12 (N2899M), Koliganek, AK, destroyed, 2 fatal. Probable cause: The pilot's failure to maintain adequate airspeed while on approach to land, which resulted in an inadvertent stall and an uncontrolled descent.
