## Narrative

As the FAA filed it, in capitals; the FAA replaces private details with ^PRIVACY DATA OMITTED^.

(-23) ON SEPTEMBER 9, 2004, AT APPROXIMATELY 1020 LOCAL TIME, N994RW (ROBINSON R-22 HELICOPTER) CRASHED AND WAS DESTROYED WHILE PRACTICING 180 DEGREE AUTOROTATIONAL DESCENTS TO POWER RECOVERIES. THE PILOT-IN-COMMAND (QUALIFIED FLIGHT INSTRUCTOR), ANDREW R. ISOM, WAS FLYING N994RW WHEN THE ACCIDENT OCCURRED. THIS CFI TOOK CONTROL OF THE MANEUVER WHEN THE STUDENT APPLIED INCORRECT THROTTLE APPLICATION. DURING THE 180 TURN N994RW WAS APPARENTLY ENTERED INTO A HIGH RATE OF DESCENT AND THIS CFI WAS UNABLE TO GAIN CONTROL BEFORE IMPACTING RUNWAY 12L AT VGT. NTSB AUTHORIZED REMOVAL OF N994RW TO A SECURE LOCATION AND WAS INSPECTED BY AN AW INSPECTOR FROM LAS.

## The NTSB's investigation of this event (1)

From the NTSB's public API. Aircraft are matched to registry aircraft by N-number and event date, checked against the reported serial number, make and model; the registry's history has gaps, so older matches can be wrong. Compare the reported make and model.

- [NTSB LAX04LA314](https://openairdocs.org/ntsb/LAX04LA314), Sep 5, 2004: Robinson R22 Beta (N994RW), North Las Vegas, NV, substantial damage. Probable cause: The certified flight instructor's misjudged flare and delayed recovery from the practice autorotation.

From the FAA Accident/Incident Data System. Reports are matched to registry aircraft by N-number and event date, checked against the reported make and model; the registry's history has gaps, so older matches can be wrong. Compare the reported make and model. For accidents, the NTSB's own investigation is the fuller record.
