## Narrative

As the FAA filed it, in capitals; the FAA replaces private details with ^PRIVACY DATA OMITTED^.

(-23) ON SEPTEMBER 17, 2004 MR. JAMES (CFI) AND MR. HILL (STUDENT) WERE PRACTICING AUTOROTATIONS AT KONA AIRPORT. AIRCRAFT ENTERED A PRACTICE AUTO AND AT 200 FEET CFI CALLED FOR GO-AROUND. AIRCRAFT AIRSPEED WAS LOW PLUS LOW ROTOR HORN BEGAN TO SOUND. CFI FAILED TO ROLL THROTTLE TO FLIGHT POSITION AND STUDENT WAS NOT FAMILIAR WITH GO-AROUND PROCEDURE. AIRCRAFT LANDED IN A LEVEL POSITION, SPREAD SKIDS (HARD LANDING) AND CHOPPED OFF TAIL BOOM AND TAIL ROTOR.

## The NTSB's investigation of this event (1)

From the NTSB's public API. Aircraft are matched to registry aircraft by N-number and event date, checked against the reported serial number, make and model; the registry's history has gaps, so older matches can be wrong. Compare the reported make and model.

- [NTSB LAX04CA321](https://openairdocs.org/ntsb/LAX04CA321), Sep 18, 2004: Robinson R22 Beta (N22XM), Kona, HI, substantial damage. Probable cause: the CFI's failure to maintain an adequate airspeed and main rotor rpm, and his improper flare, which resulted in a hard landing. A factor in the accident was the CFI's diverted attention.

From the FAA Accident/Incident Data System. Reports are matched to registry aircraft by N-number and event date, checked against the reported make and model; the registry's history has gaps, so older matches can be wrong. Compare the reported make and model. For accidents, the NTSB's own investigation is the fuller record.
