## Narrative

As the FAA filed it, in capitals; the FAA replaces private details with ^PRIVACY DATA OMITTED^.

(-23) PILOT/BUILDER WAS COMPLETING THE SECOND FLIGHT OF HOMEBUILT AMPHIBIOUS AIRPLANE. AIRCRAFT WAS FLYING A STANDARD LEFT HAND TRAFFIC PATTERN TO RUNWAY 23 AT NORTH CENTRAL STATE AIRPORT, RI. WITNESSES STATED THAT THE AIRCRASFT SEEMED A LITTLE SLOW AND LOW ON ITS BASE TO FINAL TURN. EYEWITNESSES STATED THAT THE NOSE OF THE AIRCRAFT PITCHED UP SLIGHTLY AND THEN PITCHED DOWN. THE AIRCRAFT THEN DESCENDED STRAIGHT DOWN AND MADE A ONE-QUARTER TO ONE-HALF TURN PRIOR TO CRASHING UPSIDE DOWN. THE AIRCRAFT BURST INTO FLAMES ON IMPACT AND WAS TOTALLY CONSUMED BY FIRE. ^PRIVACY DATA OMITTED^, THE PILOT WHO WAS THE SOLE PERSON ON BOARD, PERISHED IN THE CRASH.

## The NTSB's investigation of this event (1)

From the NTSB's public API. Aircraft are matched to registry aircraft by N-number and event date, checked against the reported serial number, make and model; the registry's history has gaps, so older matches can be wrong. Compare the reported make and model.

- [NTSB NYC05LA012](https://openairdocs.org/ntsb/NYC05LA012), Oct 29, 2004: Douglas Adventure Air Ampib (N469PD), Pawtucket, RI, destroyed, 1 fatal. Probable cause: The pilot's failure to maintain aircraft control, which resulted in an inadvertent stall.

From the FAA Accident/Incident Data System. Compare the reported make and model. For accidents, the NTSB's own investigation is the fuller record.
