## Narrative

As the FAA filed it, in capitals; the FAA replaces private details with ^PRIVACY DATA OMITTED^.

(-23) AT 2045L, 18 NOVEMBER, 2004, AIR WISCONSIN AIRCRAFT N451AW, FLIGHT #8368 FROM TULSA TO ORD WAS TAXIING TO GATE F3A, WHEN THE RIGHT WING OUTBOARD LEADING EDGE COLLIDED WITH A DODGE MINIVAN BELONGING TO WORLDWIDE SERVICES. THE COLLISION CAUSED THE MINIVAN TO FLIP OVER ON THE PASSENGER SIDE. THREE OCCUPANTS WERE ONBOARD THE VAN AND WERE INJURED. THE INJURED WERE TAKEN TO LUTHERAN GENERAL AND ILLINOIS MASONIC HOSPITALS. THE AIR WISCONSIN CREW REPORTED FEELING AN IMPACT, BUT DID NOT SEE ANYTHING FROM THE COCKPIT. THE AIRCRAFT WAS INSTRUCTED BY OPERATION TO CONTINUE TAXIING TO GATE F3A. NO INJURIES WERE REPORTED ONBOARD THE AIRCRAFT. THE PASSENGERS WERE DISEMBARKED WITHOUT INCIDENT. THE AIRCRAFT SUSTAINED DAMAGE TO THE RIGHT WING LEADING EDGE, OUTBOARD FLAP CANOE FAIRING AND LOWER WING SKIN PANEL. THE MINIVAN WAS TOTALLED TO THE IMPACT.

From the FAA Accident/Incident Data System. Reports are matched to registry aircraft by N-number and event date, checked against the reported make and model; the registry's history has gaps, so older matches can be wrong. Compare the reported make and model. For accidents, the NTSB's own investigation is the fuller record.
