## Narrative

As the FAA filed it, in capitals; the FAA replaces private details with ^PRIVACY DATA OMITTED^.

(-23) ON JANUARY 19, 2005, U.S. AIRWAYS EXPRESS (OPERATED BY MESA AIRLINES), FLIGHT 2814 RETURNED TO LEXINGTON, KY (LEX) DUE TO SMOKE IN THE COCKPIT. FLIGHT 2814 DEPARTED LEXINGTON, KY (LEX) ENROUTE TO CHARLOTTE, NC. APPROXIMATELY 20 MINUTES INTO THE FLIGHT A FLIGHT ATTENDANT INFORMED THE CREW OF A HAZE AND ODOROUS SMELL IN THE AIRCRAFT. THE AIRCRAFT RETURNED TO LEX AND LANDED SAFELY AND WITHOUT INCIDENT. THERE WERE NO INJURIES REPORTED. THERE WERE 23 OCCUPANTS ON THE AIRCRAFT. THE AIRCRAFT WAS INSPECTED BY LEX MAINTENANCE PERSONNEL AND DE-ICE FLUID WAS FOUND IN THE ENVIRONMENTAL DUCTING. THE AIRCRAFT WAS FERRIED ON A NON-REVENUE FLIGHT TO CHARLOTTE MAINTENANCE FOR FURTHER INSPECTION. UPON ARRIVAL AT CHARLOTTE MAINTENANCE REPLACED THE NUMBER 2 AIR CONDITIONING PACK.

From the FAA Accident/Incident Data System. Reports are matched to registry aircraft by N-number and event date, checked against the reported make and model; the registry's history has gaps, so older matches can be wrong. Compare the reported make and model. For accidents, the NTSB's own investigation is the fuller record.
