## Narrative

As the FAA filed it, in capitals; the FAA replaces private details with ^PRIVACY DATA OMITTED^.

(-23) AIRCRAFT DEPARTED BLOOMFIELD MUNICIPAL AIRPORT AT APPROXIMATELY 09:30 ON RUNWAY 18. AN EYEWITNESS STATED THAT JUST AS THE AIRCRAFT TOOK OFF, THE OIL DOOR ON THE TOP OF THE COWLING POPPED OPEN. THE AIRCRAFT WAS OBSERVED TO MAKE A SHALLOW TURN TO THE RIGHT AND PROCEED IN A SOUTHWESTERLY DIRECTION BUT DID NOT CLIMB ABOVE 100 OR 200 FEET AGL. THREE WITNESSES OBSERVED THE AIRCRAFT IN A LEFT TURN IN WHAT APPEARED TO BE AN ATTEMPT TO RETURN TO THE AIRPORT. TWO OF THE WITNESSES CONTINUED TO OBSERVE THE AIRCRAFT AS IT TURNED AND STATED THAT THE AIRCRAFT ABRUPTLY WENT HARD OVER RESULTING IN THE WINGS BEING VERTICAL TO THE GROUND THEN WENT INTO A NOSE DIVE. A FOURTH WITNESS'S ATTENTION WAS DRAWN TO THE AIRCRAFT DUE TO A SUDDEN APPLICATION OF POWER, AT WHICH TIME THIS WITNESS DESCRIBED THE SAME AIRCRAFT MOVEMENTS AS THE OTHER WITNESSES. THE AIRCRAFT IMPACTED THE GROUND IN A STEEP NOSE-DOWN ATTITUDE APPROXIMATELY ONE-HALF MILE SOUTH OF BLOOMFIELD AIRPORT. THE WRECKAGE CAME TO REST 69.5 FEET FROM THE INITIAL IMPACT. THE PASSENGER RECEIVED FATAL INJURIES; THE PILOT SERIOUS INJURIES. AIRCRAFT WAS DESTROYED.

## The NTSB's investigation of this event (1)

From the NTSB's public API. Aircraft are matched to registry aircraft by N-number and event date, checked against the reported serial number, make and model; the registry's history has gaps, so older matches can be wrong. Compare the reported make and model.

- [NTSB CHI05LA083](https://openairdocs.org/ntsb/CHI05LA083), Mar 28, 2005: Mooney M20E (N6092Q), Bloomfield, IA, destroyed, 1 fatal. Probable cause: The pilot's failure to maintain airspeed which resulted in an inadvertent stall/spin. The low altitude was a factor.

## Other reports on N6092Q (1)

Matched to the same aircraft by N-number and date.

- [Incident 19891107044699I](https://openairdocs.org/incident/19891107044699I), Nov 7, 1989: Mooney M20E (N6092Q), Bloomfield, IA, Minor damage. Pilot forgot to lower landing gear and landed with gear retracted.

From the FAA Accident/Incident Data System. Reports are matched to registry aircraft by N-number and event date, checked against the reported make and model; the registry's history has gaps, so older matches can be wrong. Compare the reported make and model. For accidents, the NTSB's own investigation is the fuller record.
