## Narrative

As the FAA filed it, in capitals; the FAA replaces private details with ^PRIVACY DATA OMITTED^.

(-23) THE PILOT WAS MAKING HIS INITIAL APPROACH INTO AN ALFALFA FIELD FOR THE PURPOSES OF AERIAL APPLICATION OF A LIQUID CARBAMATE LABELED AS "FURADAN 4F". DURING INITIAL APPROACH TO THE FIELD, THE AIRCRAFT STRUCK TREE BRANCHES BORDERING THE FIELD AND SUBSEQUENTLY IMPACTED THE GROUND. THE PILOT RECEIVED FATAL INJURIES AND THE AIRCRAFT WAS SUBSTANTIALLY DAMAGED.

## The NTSB's investigation of this event (1)

From the NTSB's public API. Aircraft are matched to registry aircraft by N-number and event date, checked against the reported serial number, make and model; the registry's history has gaps, so older matches can be wrong. Compare the reported make and model.

- [NTSB CHI05LA114](https://openairdocs.org/ntsb/CHI05LA114), May 14, 2005: Cessna A188B (N4734Q), Ludell, KS, destroyed, 1 fatal. Probable cause: The pilot's failure to maintain clearance from trees during an aerial application maneuver. The trees and the sun glare were contributing factors.

From the FAA Accident/Incident Data System. Reports are matched to registry aircraft by N-number and event date, checked against the reported make and model; the registry's history has gaps, so older matches can be wrong. Compare the reported make and model. For accidents, the NTSB's own investigation is the fuller record.
