## Narrative

As the FAA filed it, in capitals; the FAA replaces private details with ^PRIVACY DATA OMITTED^.

(-23) THE PILOT WAS PRACTICING A TAKEOFF AND LANDING AT A REMOTE SITE. THE INSTRUCTOR SAID HE THEN INSTRUCTED THE STUDENT TO PRACTICE HOVERING. HE SAID THE STUDENT LOST CONCENTRATION, AND BEGAN TO PANIC. THE INSTRUCTOR THEN HAD THE STUDENT PRACTICE HOVERING WITH ONE CONTROL AT A TIME WHILE THE INSTRUCTOR HAD THE OTHER CONTROLS. IMMEDIATELY PRIOR TO THE ACCIDENT THE STUDENT HAD CONTROL OF THE CYCLIC LEVER. THE INSTRUCTOR DIRECTED THE STUDENT TO APPLY LEFT CYCLIC, BUT THE STUDENT APPLIED RIGHT AND REAR CYCLIC INSTEAD AND THE HELICOPTER BEGAN TO MOVE AFT AND TO THE RIGHT. THE INSTRUCTOR SAID HE THEN APPLIED FORWARD AND LEFT CYCLIC TO LEVEL THE AIRCRAFT. THE AIRCRAFT HIT THE GROUND WITH THE RIGHT SKID AND THE INSTRUCTOR DECREASED COLLECTIVE AND INCREASED LEFT CYCLIC TO THE MAXIMUM. THE AIRCRAFT DID A DYNAMIC ROLL OVER.

## The NTSB's investigation of this event (1)

From the NTSB's public API. Aircraft are matched to registry aircraft by N-number and event date, checked against the reported serial number, make and model; the registry's history has gaps, so older matches can be wrong. Compare the reported make and model.

- [NTSB DEN05LA082](https://openairdocs.org/ntsb/DEN05LA082), May 23, 2005: Robinson R22 Beta (N553SH), Provo, UT, substantial damage. Probable cause: the dual student’s failure to maintain adequate rotor rpm and the inadequate supervision by the instructor.

From the FAA Accident/Incident Data System. Reports are matched to registry aircraft by N-number and event date, checked against the reported make and model; the registry's history has gaps, so older matches can be wrong. Compare the reported make and model. For accidents, the NTSB's own investigation is the fuller record.
