## Narrative

As the FAA filed it, in capitals; the FAA replaces private details with ^PRIVACY DATA OMITTED^.

(-23) ^PRIVACY DATA OMITTED^ LEFT OUTBOARD MLG WHEEL AND TIRE ASSEMBLY SEPARATED FROM AIRCRAFT ON TAKE OFF ROLL. CREW HAD NO INDICATION OF PROBLEM UNTIL POST FLIGHT WALK AROUND AT DESTINATION. FAIRBANKS AIRPORT MAINTENANCE PERSONNEL FOUND WHEEL AND TIRE ASSEMBLY BESIDE RUNWAY DURING A ROUTINE CHECK AND RETURNED TO CARRIER. FAA INSPECTORS AND CARRIER MAINTENANCE PERSONNEL INSPECTED ASSEMBLY AND DETERMINED THAT OUTER BEARING HAD FAILED, ALLOWING THE WHEEL TO SEPARATE OVER AXLE NUT. CARRIER MAINTENANCE PERSONNEL WERE DISPATCHED TO OTZ WHERE THEY CONFIRMED THAT AXLE NUT AND COTTER KEY WERE STILL IN PLACE. THERE WAS NO DAMAGE TO THE AXLE, BRAKE ASSEMBLY OR AIRFRAME. THE WHEEL AND TIRE ASSEMBLY HAD ACCUMULATED 386 HOURS AND 364 LANDINGS SINCE BUILD-UP, BEARING LUBRICATION, AND INSTALLATION.

From the FAA Accident/Incident Data System. Reports are matched to registry aircraft by N-number and event date, checked against the reported make and model; the registry's history has gaps, so older matches can be wrong. Compare the reported make and model. For accidents, the NTSB's own investigation is the fuller record.
