## Narrative

As the FAA filed it, in capitals; the FAA replaces private details with ^PRIVACY DATA OMITTED^.

(-23) ON 9/21/2005, AT APPROXIMATELY 2200 HOURS PDT, HILLER UH-12E HELICOPTER, N141HA, CRASHED APPROXIMATELY 12.5 MILES WEST OF FIVE POINTS, CA. N141HA, OPERATED BY HURON AG HELICOPTERS, WAS CONDUCTING AGRICULTURAL AERIAL APPLICATION OPERATIONS UNDER 14 CFR 137 IN VMC CONDITIONS. THE PILOT ^PRIVACY DATA OMITTED^ COMPLETED SPRAYING A LOAD OF ORGANIC BIOLOGICAL PESTICIDE AND PULLED UP IN A LEFT PEDAL TURN TO RETURN TO THE SERVICE TRUCK FOR REFUELING AND RELOADING. DURING THE ZERO AIRSPEED TURN AT 75' AGL, THE ENGINE LOST POWER DUE TO FUEL STARVATION. THE PIC LEVELED THE HELICOPTER WITH CYCLIC CONTROL AND ENTERED AUTOROTATION CAUSING A SKIDS LEVEL HARD LANDING AT ZERO FORWARD SPEED. THE HELICOPTER BOUNCED APPROXIMATELY TWO FEET HIGH AT GROUND IMPACT CAUSING THE MAIN ROTOR BLADES TO FLEX DOWN AND SEVER THE TAIL BOOM. THE HELICOPTER ROTATED VIOLENTLY TO THE RIGHT AND IMPACTED THE GROUND A SECOND TIME COMING TO REST ON THE RIGHT SKID SUPPORTED BY THE MAIN ROTOR BLADE. THE PILOT WAS NOT INJURED, AND NO SPILLAGE OF PESTICIDE OCCURRED.

## The NTSB's investigation of this event (1)

From the NTSB's public API. Aircraft are matched to registry aircraft by N-number and event date, checked against the reported serial number, make and model; the registry's history has gaps, so older matches can be wrong. Compare the reported make and model.

- [NTSB LAX05CA313](https://openairdocs.org/ntsb/LAX05CA313), Sep 22, 2005: Hiller UH 12-E (N141HA), Five Points, CA, substantial damage. Probable cause: The pilot's failure to refuel the helicopter, which resulted in fuel exhaustion.

From the FAA Accident/Incident Data System. Reports are matched to registry aircraft by N-number and event date, checked against the reported make and model; the registry's history has gaps, so older matches can be wrong. Compare the reported make and model. For accidents, the NTSB's own investigation is the fuller record.
