## Narrative

As the FAA filed it, in capitals; the FAA replaces private details with ^PRIVACY DATA OMITTED^.

(-23) THIS WAS A TRAINING FLIGHT. THE INSTRUCTOR HAD THE STUDENT ENTER A 180 DEGREE PRACTICE AUTOROTATION FROM 500' AGL. THE MANEUVER WAS TO END WITH A POWER RECOVERY. THE AIRCRAFT EXPERIENCED A HARD LANDING ON RUNWAY 4 AT MUE. THE HARD LANDING CAUSED THE MAIN ROTOR TO FLEX DOWN AND SLICE THROUGH THE TAIL BOOM JUST FORWARD OF THE TAIL ROTOR GEAR BOX. THE TAIL ROTOR AND PART OF THE TAIL BOOM SEPARATED FROM THE AIRCRAFT ON THE INITIAL IMPACT. THE AIRCRAFT BOUNCED FROM THE INITIAL IMPACT WHICH PLACED IT IN THE AIR WITHOUT THE TAIL ROTOR. AFTER THE SECOND GROUND CONTACT THE AIRCRAFT SPUN 7 AND ONE QUARTER TIMES BEFORE COMING TO REST ON THE RUNWAY JUST LEFT OF CENTERLINE.

## The NTSB's investigation of this event (1)

From the NTSB's public API. Aircraft are matched to registry aircraft by N-number and event date, checked against the reported serial number, make and model; the registry's history has gaps, so older matches can be wrong. Compare the reported make and model.

- [NTSB LAX05CA314](https://openairdocs.org/ntsb/LAX05CA314), Sep 23, 2005: Robinson R22 Beta (N82001), Kamuela, HI, substantial damage. Probable cause: The student's misjudged landing flare, and the instructor's inadequate supervision and delayed remedial action. A factor in the accident was the high density altitude.

From the FAA Accident/Incident Data System. Reports are matched to registry aircraft by N-number and event date, checked against the reported make and model; the registry's history has gaps, so older matches can be wrong. Compare the reported make and model. For accidents, the NTSB's own investigation is the fuller record.
