## Narrative

As the FAA filed it, in capitals; the FAA replaces private details with ^PRIVACY DATA OMITTED^.

(-23) ON NOVEMBER 23, 2005 AT APPROXIMATELY 1200 LOCAL TIME, A PIPER PA-23, N859DA REGISTERED TO ^PRIVACY DATA^ AND PILOTED BY COMMERCIAL MULTI ENGINE INSTRUMENT RATED PILOT ^PRIVACY DATA^ DEPARTED RVS UNDER VMC CONDITIONS. SHORTLY AFTER TAKEOFF THE PILOT REPORTED THAT BOTH ENGINES BEGAN TO LOSE POWER AND THAT THE AIRCRAFT WOULD NOT MAINTAIN ALTITUDE. THE PILOT PERFORMED AN EMERGENCY LANDING IN A HAY MEADOW APPROXIMATELY 6 MILES SOUTH OF RVS. THE AIRCRAFT WAS SUBSTANTIALLY DAMAGED WHEN THE NOSE LANDING GEAR COLLAPSED DUE TO ROUGH TERRAIN. NEITHER THE PILOT NOR THE PASSENGER WERE INJURED.

## The NTSB's investigation of this event (1)

From the NTSB's public API. Aircraft are matched to registry aircraft by N-number and event date, checked against the reported serial number, make and model; the registry's history has gaps, so older matches can be wrong. Compare the reported make and model.

- [NTSB DFW06CA033](https://openairdocs.org/ntsb/DFW06CA033), Nov 23, 2005: Piper PA-23-250 (N859DA), Bixby, OK, substantial damage. Probable cause: The loss of engine power due to water contamination and corrosion in both gascolators due to improper maintenance. A factor was the lack of suitable terrain for the forced landing.

From the FAA Accident/Incident Data System. Reports are matched to registry aircraft by N-number and event date, checked against the reported make and model; the registry's history has gaps, so older matches can be wrong. Compare the reported make and model. For accidents, the NTSB's own investigation is the fuller record.
