## Narrative

As the FAA filed it, in capitals; the FAA replaces private details with ^PRIVACY DATA OMITTED^.

(-23) THE PILOT ^PRIVACY DAT^ STATES THAT WHILE PRACTICING TAXIING THE TANDEM SEAT AIRCRAFT FROM THE AFT POSITION, HE "IMPULSIVELY" DECIDED TO TAKE-OFF FROM PHILLIPS FIELD AIRPORT (MO23), CALIFORNIA, MISSOURI. SHORTLY AFTER TAKE-OFF HE NOTICED A NOSE HIGH ATTITUDE WHICH HE TRIED TO CORRECT WITH FULL FORWARD STICK MOVEMENT AND FLAPERON TRIM. AT THIS TIME A DECISION WAS MADE TO RETURN TO MO23 AND LAND. THE PILOT ^PRIVACY DAT^ STATES THAT WHILE TURNING TO BASE HE "WAS STILL TRYING TO TRIM THE PLANE AND LOST ATTENTION TO AIRSPEED." SHORTLY THEREAFTER, AT ABOUT 400 FEET AGL, THE AIRCRAFT ENTERED INTO AN UNRECOVERABLE STALL-SPIN SITUATION, IMPACTING THE GROUND APPROXIMATELY 1/2 MILE NORTHEAST OF MO23. THE AIRCRAFT WAS DESTROYED, AND THE PILOT ^PRIVACY DAT^ RECEIVED MINOR INJURIES. ^PRIVACY DATA OMITT^

## The NTSB's investigation of this event (1)

From the NTSB's public API. Aircraft are matched to registry aircraft by N-number and event date, checked against the reported serial number, make and model; the registry's history has gaps, so older matches can be wrong. Compare the reported make and model.

- [NTSB CHI06CA050](https://openairdocs.org/ntsb/CHI06CA050), Dec 21, 2005: Elliott Quad City Ultralight Challenger II (N115BT), California, MO, substantial damage. Probable cause: The pilot's failure to maintain aircraft control due to his failure to comply with procedures/directives, and the inadvertent stall/spin he encountered on his turn to base.

From the FAA Accident/Incident Data System. Reports are matched to registry aircraft by N-number and event date, checked against the reported make and model; the registry's history has gaps, so older matches can be wrong. Compare the reported make and model. For accidents, the NTSB's own investigation is the fuller record.
