## Narrative

As the FAA filed it, in capitals; the FAA replaces private details with ^PRIVACY DATA OMITTED^.

(-23) THE PILOT ^PRIVACY DAT^ IS A PVT. PILOT WITH SEL, MEL, INSTRUMENT AIRPLANE, AND ROTORCRAFT RATINGS. THE PILOT ^PRIVACY DAT^ SUFFERED AN INITIAL ACCIDENT IN HIS R44 HELICOPTER ON 25 JULY 2005 AND WAS SENT A 44709 LETTER REGARDING HIS REEXAMINATION FOR COMPETENCY IN HELICOPTERS. HE REQUESTED A DELAY PENDING REPAIR OF HIS HELICOPTER AND WAS GRANTED IT. HIS HELICOPTER WAS FINALLY REPAIRED IN JAN OF 2006 BUT NEEDED FIVE HOURS OF "RUN TIME" BEFORE FLYING BEYOND THE LOCAL FLYING AREA. THE PILOT ^PRIVACY DAT^ WAS IN THE PROCESS OF ACCUMULATING THE FIVE HOURS WHEN HE SUFFERED ANOTHER ACCIDENT IN THE SAME HELICOPTER. THE ACCIDENT WAS A CARBON COPY OF THE EARLIER ACCIDENT IN THAT DURING TAKEOFF, THE HELICOPTER SUFFERED A LOSS OF MAIN ROTOR RPM FOLLOWED BY THE LOW RPM AUDIO WARNING, FOLLOWED BY A LOSS OF TAIL ROTOR AUTHORITY AND LOSS OF DIRECTIONAL CONTROL, FOLLOWED BY A CRASH LANDING BY THE PILOT ^PRIVACY DAT^ WITH SIGNIFICANT DAMAGE DONE TO THE HELICOPTER BUT NO INJURIES TO PEOPLE ON BOARD.
