## Narrative

As the FAA filed it, in capitals; the FAA replaces private details with ^PRIVACY DATA OMITTED^.

(-23) UNREGISTERED AIRCRAFT ACCIDENT--THE ALLEGHENY FSDO HAS RECLASSIFIED THE MISCELLANEOUS INCIDENT REPORTED IN THE 5/8/06 DAILY OPERATIONS REPORT INVOLVING AN UNREGISTERED ULTRALIGHT VEHICLE AT THE WEST MIDDLESEX AIRPORT. THE EXPERIMENTAL AIRCRAFT INVOLVED WAS AN UNREGISTERED GT-400 QUICKSILVER, AND THE PILOT OF THE AIRCRAFT, WHO SUSTAINED SERIOUS INJURIES, WAS NON-RATED FOR FLIGHT. FURTHER INVESTIGATION BY THIS OFFICE ON 5/10/06, REVEALED THAT ^PRIVACY DATA^ THE NON-RATED PILOT CRASHED HIS GT-400 AIRCRAFT ON TAKE OFF. THE TAKE OFF WAS MADE FROM RUNWAY 36 AND CRASHED 1,056 FEET FROM THE APPROACH END OF RUNWAY 36. THE AIRCRAFT WAS FOUND POINTING IN A DIRECTION OF APPROXIMATELY 200 DEGREES AS REPORTED BY WITNESS ^PRIVACY DATA O^. THE AIRCRAFT HAD BEEN REMOVED FROM THE CRASH SITE AND PUT INTO A HANGAR, PRIOR TO THE ARRIVAL OF THE FAA INVESTIGATORS. IT WAS, HOWEVER, DETERMINED THAT GROUND SCARS OF THE CRASH WERE LOCATED 60 FEET FROM THE RIGHT EDGE OF RUNWAY 36, HEADING EAST. IT WAS ALSO NOTED THAT TREE BRANCHES WERE SCORED APPROXIMATELY 50 FEET ABOVE THE GROUND, AND TREE LEAVES WERE FOUND ON THE RIGHT STRUT OF THE GT-400 AIRCRAFT.

## NTSB investigation

None matched to this report by aircraft and date. Search the NTSB's own records: https://data.ntsb.gov/carol-main-public/basic-search

From the FAA Accident/Incident Data System. Reports are matched to registry aircraft by N-number and event date, checked against the reported make and model; the registry's history has gaps, so older matches can be wrong. Compare the reported make and model. For accidents, the NTSB's own investigation is the fuller record.
