## Narrative

As the FAA filed it, in capitals; the FAA replaces private details with ^PRIVACY DATA OMITTED^.

(-23) ON JUNE 8, 2006, AT 1230 CDT, THE PILOT IN COMMAND OF N4035G, AN FH-1100 HELICOPTER DEPARTED FROM A PRIVATE HELIPORT AT NAVARRE BEACH FLORIDA ON A PERSONAL FLIGHT. SHORTLY AFTER TAKEOFF, THE PILOT HEARD A LOUD BANG AND FELT A SLIGHT SHUDDER IN THE AIRFRAME. HE INITIATED AN IMMEDIATE DESCENT AND PRIOR TO LANDING DISCOVERED HE HAD LOST DIRECTIONAL CONTROL, AT WHICH TIME HE REDUCED POWER TO IDLE AND PERFORMED AN AUTOROTATIONAL LANDING TO THE BEACH. ONE OF THE TAIL ROTOR BLADES DEPARTED THE AIRCRAFT IN FLIGHT AND COULD NOT BE LOCATED. THE RESULTING OUT OF BALANCE CONDITION CAUSED THE REMAINDER OF THE TAIL ROTOR AND THE NINETY DEGREE GEARBOX TO COME OFF OF ITS MOUNTS AND REMAINED SUSPENDED FROM THE TAIL BY THE CONTROL CABLE. THE AIRCRAFT WAS LANDED SUCCESSFULLY WITH NO FURTHER DAMAGE. INVESTIGATION CONTINUES TO DETERMINE CAUSE OF TAIL ROTOR FAILURE.

## The NTSB's investigation of this event (1)

From the NTSB's public API. Aircraft are matched to registry aircraft by N-number and event date, checked against the reported serial number, make and model; the registry's history has gaps, so older matches can be wrong. Compare the reported make and model.

- [NTSB MIA06LA126](https://openairdocs.org/ntsb/MIA06LA126), Jun 8, 2006: Fairchild Hiller FH-1100 (N4035G), Navarre, FL, substantial damage. Probable cause: The inadequate manufacturing of the plates of the tail rotor tension-torsion bar assembly resulting in fatigue failure.

From the FAA Accident/Incident Data System. Reports are matched to registry aircraft by N-number and event date, checked against the reported make and model; the registry's history has gaps, so older matches can be wrong. Compare the reported make and model. For accidents, the NTSB's own investigation is the fuller record.
