## Narrative

As the FAA filed it, in capitals; the FAA replaces private details with ^PRIVACY DATA OMITTED^.

(-23) ON 06/11/06 AT 0730 MST, A FRONTIER AIRLINES (F3LA), FLT #580, AN AIRBUS A319-111, N929FR, RECEIVED MINOR DAMAGE TO THE RADOME OF THE AIRCRAFT. THE AIRCRAFT WAS IN PROCESS OF BEING PUSHED BACK FROM GATE A7 AT (SLC). THE PUSH WAS COMPLETED, THE TOW VEHICLE DRIVER, (CREW COORDINATOR), FAILED TO TELL THE FLIGHT CREW TO SET THE BRAKES. THE TOW MEMBER STATIONED ON THE RH WING CAME OVER AND WITHOUT PERMISSION FROM THE COORDINATOR DISCONNECTED THE TOW BAR. THE NUMBER 1 ENGINE HAD ALREADY BEEN STARTED, AND THE AIRCRAFT STARTED TO ROLL FORWARD, THE TOW TRUCK DRIVER ALERTED THE CREW, AND THEY SET THE BRAKES HARD, THE NOSE OF THE AIRCRAFT BOUNCED DOWN AND MADE CONTACT WITH THE ROOF OF THE TUG. NO INJURIES WERE REPORTED. THE TOW TEAM IS CONTRACTED TO MILLIONAIRE.

From the FAA Accident/Incident Data System. Reports are matched to registry aircraft by N-number and event date, checked against the reported make and model; the registry's history has gaps, so older matches can be wrong. Compare the reported make and model. For accidents, the NTSB's own investigation is the fuller record.
