## Narrative

As the FAA filed it, in capitals; the FAA replaces private details with ^PRIVACY DATA OMITTED^.

(-23) ON JUNE 18, 2006 A GROUP OF FOUR AIRCRAFT DEPARTED THE POPLAR GROVE AIRPORT (C77) FOR A FLIGHT TO THE ILLINOIS VALLEY AIRPORT IN PERU IL (VYS). PRIOR TO DEPARTURE THE GROUP HAD A PREFLIGHT BRIEFING DISCUSSING THE PLANNED DIAMOND FORMATION FLYING TO TAKE PLACE AT VYS. THIS BRIEFING ALSO INCLUDED HOW THE RECOVERY FOR LANDING WOULD BE COMPLETED WHILE IN THE DIAMOND FORMATION AT VYS. WHEN ARRIVING AT VYS AND ESTABLISHED IN A DIAMOND FORMATION THE AIRCRAFT CROSSED OVER THE APPROACH END OF RUNWAY 18 TO BEGIN THEIR RECOVERY FOR LANDING. THE LEAD AIRCRAFT (N68LT) BEGAN HIS RECOVERY TO LANDING BY INFORMING THE THREE AIRCRAFT IN THE GROUP HE WAS PULLING, HE THEN BEGAN A CLIMBING LEFT TURN. ACCORDING TO EYE WITNESSES LOCATED AT THE AIRPORT THE NUMBER TWO AIRCRAFT (86CG) ALSO BEGAN A CLIMB AT THE SAME TIME RESULTING IN A MID-AIR COLLISION. THE LEAD AIRCRAFT SUSTAINED SUBSTANTIAL DAMAGE BUT WAS ABLE TO RECOVER AFTER THE MID-AIR COLLISION AND CONTINUE TO A SAFE LANDING. THE NUMBER TWO AIRCRAFT'S RIGHT WING APPEARS TO HAVE FOLDED TO THE SIDE OF THE AIRCRAFT AS A RESULT OF THE MID-AIR, IT THEN BEGAN A SPIRALING DESCENT AND WAS UNABLE TO RECOVER. THE AIRCRAFT IMPACTED THE GROUND FATALLY INJURING THE PILOT.

## The NTSB's investigation of this event (1)

From the NTSB's public API. Aircraft are matched to registry aircraft by N-number and event date, checked against the reported serial number, make and model; the registry's history has gaps, so older matches can be wrong. Compare the reported make and model.

- [NTSB CHI06LA157](https://openairdocs.org/ntsb/CHI06LA157), Jun 18, 2006: Turner RV-8 (N68LT), Peru, IL, substantial damage, 1 fatal. Probable cause: Failure of the pilot of the second airplane in the formation to maintain clearance with the lead airplane during the overhead recovery maneuver prior to landing.

From the FAA Accident/Incident Data System. Reports are matched to registry aircraft by N-number and event date, checked against the reported make and model; the registry's history has gaps, so older matches can be wrong. Compare the reported make and model. For accidents, the NTSB's own investigation is the fuller record.
