## Narrative

As the FAA filed it, in capitals; the FAA replaces private details with ^PRIVACY DATA OMITTED^.

(-23) DURING TAKEOFF, RUNWAY 25 @ N96, AIRCRAFT ROTATED AND FLEW IN GROUND EFFECT OVER RUNWAY SURFACE. PILOT STATED THAT NORMAL ENGINE INDICATIONS WERE PRESENT PRIOR TO LIFT OFF. PILOT STATED AIRCRAFT DID NOT ESTABLISH POSITIVE RATE OF CLIMB UNTIL LANDING GEAR WAS RETRACTED. PILOT THEN ELECTED TO RETRACT FLAPS FROM 10 DEGREES SETTING. AIRCRAFT IMMEDIATELY SETTLED TO GROUND IN TAIL LOW POSITION AT DEPARTURE END OF RUNWAY AND PILOT RETARDED ENGINE POWER TO IDLE. AIRCRAFT CONTACTED THE GROUND IN TAIL LOW POSITION AND SLID THROUGH AIRPORT PERIMETER FENCE AND CAME TO REST ON A HEADING OF APPROXIMATELY 130 DEGREES IN GRASSY FIELD, APPROXIMATELY 130 YDS FROM RUNWAY 25 DEPARTURE END. PILOT AND FIVE PASSENGERS EGRESSED FROM AIRCRAFT WITHOUT INJURIES. POST ACCIDENT FIRE IMMEDIATELY OCCURRED AFTER EGRESS WAS COMPLETED, WHICH DESTROYED THE COCKPIT AND CABIN STRUCTURE, ENTIRE RIGHT WING STRUCTURE AND PORTIONS OF THE LEFT WING STRUCTURE. ENGINE COMPARTMENT AND COMPONENTS INCURRED MINIMAL HEAT DAMAGE. ENGINE HAS BEEN RETAINED FOR FURTHER INVESTIGATIN. INSPECTION OF AIRCRAFT REVEALED THE PITCH TRIM ACTUATOR/DRUM ASSEMBLY HAVING 12 THREADS EXPOSED. THIS ACTUATOR CONDITION IS CONSISTENT WITH A NEARLY FULL NOSE UP TRIM SETTING.
