## Narrative

As the FAA filed it, in capitals; the FAA replaces private details with ^PRIVACY DATA OMITTED^.

(-23) ON SEPTEMBER 28, 2006, APPROXIMATELY 18:19 CENTRAL DAYLIGHT TIME, AN AMATEUR-BUILT HELICOPTER, N565CH, ROLLED OVER DURING A HOVER. THE PILOT AND A PASSENGER WERE CHECKING THE ENGINE COMPUTER READOUTS USING AN AUTOMOTIVE SCAN TOOL. ACCORDING TO THE PILOT, WHILE IN A HOVER APPROXIMATELY TWO FEET ABOVE GROUND LEVEL, THE PILOT HEARD A "LOUD BANG" AND THE AIRCRAFT BEGAN TO DRIFT TO THE LEFT. THE PILOT ATTEMPTED TO APPLY RIGHT CYCLIC BUT THE AIRCRAFT WOULD NOT RESPOND. THE TAIL OF THE AIRCRAFT PITCHED UPWARD CAUSING THE SKID TO CONTACT THE GROUND. THE AIRCRAFT THEN ROLLED OVER ALLOWING THE ROTOR TO CONTACT THE GROUND. THE AIRCRAFT SUSTAINED SUBSTANTIAL DAMAGE DURING A POST ACCIDENT FIRE. THE PRIVATE PILOT AND PASSENGER RECEIVED MINOR INJURIES AS A RESULT OF THE ACCIDENT.

## The NTSB's investigation of this event (1)

From the NTSB's public API. Aircraft are matched to registry aircraft by N-number and event date, checked against the reported serial number, make and model; the registry's history has gaps, so older matches can be wrong. Compare the reported make and model.

- [NTSB CHI06LA273](https://openairdocs.org/ntsb/CHI06LA273), Sep 28, 2006: Daub Pawnee Chief (N565CH), McCook, NE, destroyed. Probable cause: The failure of a welded joint at a cyclic control cable bracket located on the main rotor mast which made control of the helicopter impossible for the pilot.

From the FAA Accident/Incident Data System. Reports are matched to registry aircraft by N-number and event date, checked against the reported make and model; the registry's history has gaps, so older matches can be wrong. Compare the reported make and model. For accidents, the NTSB's own investigation is the fuller record.
