## Narrative

As the FAA filed it, in capitals; the FAA replaces private details with ^PRIVACY DATA OMITTED^.

(-23) ON OCTOBER 22, 2006 AT APPROXIMATELY 0630 LCL, N22342, A SIKORSKY SK-76A++ AIRCRAFT CRASHED WHILE APPROACHING A HELIDECK FOR LANDING. THE PREVAILING CEILING AND VISIBILITY WAS 600 TO 700 FEET CEILING AND APPROXIMATELY 10 MILES VISIBILITY WITH INTERMITTENT THUNDER SHOWERS IN THE VICINITY. UPON ARRIVING ADJACENT TO THE LANDING ZONE, THE FLIGHT CREW SETUP THE LANDING BY ENTERING A LEFT BASE WITH FINAL APPROACH TO THE NORTH. DURING FINAL APPROACH THE AIRCRAFT ENTERED THE OUTER EDGE OF A RAIN SHOWER, THE RAIN BECAME MODERATE TO HEAVY AND WINDSHIELD WIPERS WERE CALLED FOR BY THE PIC. WIPERS WERE CALLED FOR THE SECOND TIME, AGAIN WITH NO RESPONSE. THE PIC DIVERTED HIS ATTENTION INSIDE THE AIRCRAFT TO ASSIST IN ACTIVATING THE WINDSHIELD WIPERS. THE AIRCRAFT CRASHED SHORTLY AFTER. A COMPANY FLIGHT PLAN WAS ON FILE AND THE ELECTRONIC FLIGHT TRACKING SYSTEM, THE OUTERLINK SYSTEM, WAS OPERATIONAL. BOTH CREW MEMBERS SUSTAINED MINOR INJURIES.

## The NTSB's investigation of this event (1)

From the NTSB's public API. Aircraft are matched to registry aircraft by N-number and event date, checked against the reported serial number, make and model; the registry's history has gaps, so older matches can be wrong. Compare the reported make and model.

- [NTSB DFW07LA011](https://openairdocs.org/ntsb/DFW07LA011), Oct 22, 2006: Sikorsky S-76A++ (N22342), Eugene Isl 259, destroyed. Probable cause: The flight crew's failure to maintain clearance with the water and their diverted attention to secondary tasks while preparing to land. Contributing factor were the low ceilings and the heavy rains…

## Other reports on N22342 (1)

Matched to the same aircraft by N-number and date.

- [Incident 19930209011769I](https://openairdocs.org/incident/19930209011769I), Feb 9, 1993: Skrsky S76A (N22342), Clute, TX, Minor damage. Experienced sparking at electical wire bundle. Landed on road. Wire bundle chafed with radar unit.

From the FAA Accident/Incident Data System. Reports are matched to registry aircraft by N-number and event date, checked against the reported make and model; the registry's history has gaps, so older matches can be wrong. Compare the reported make and model. For accidents, the NTSB's own investigation is the fuller record.
