## Narrative

As the FAA filed it, in capitals; the FAA replaces private details with ^PRIVACY DATA OMITTED^.

(-23) HELICOPTER DEPARTED DEKALB AIRPORT (DKB) FOR A LOCAL FLIGHT. AT APPROXIMATELY 7 MILES WEST OF DKB A YELLOW ENGINE WARNING LIGHT CAME ON THE FADEC DISPLAY INDICATING A CODE #2 WARNING AND THE PILOT DECIDED TO MAKE A PRECAUTIONARY LANDING IN A CORN FIELD 1 1/2 MILES WEST OF MALTA, IL. IT WAS DETERMINED THAT THE FADEC CODE WAS TRIGGERED BY A MOMENTARY OVERSPEED OF ENGINE RPM AND AFTER LETTING WATER AND OIL TEMPERATURES STABILIZE, THE PILOT ATTEMPTED TO TAKEOFF. AS PILOT RAISED THE COLLECTIVE, THE RIGHT SKID ROSE OFF THE GROUND BUT THE LEFT SKID WAS STUCK IN THE MUD AND THE HELICOPTER ABRUPTLY ROLLED OVER ONTO THE LEFT SKID AND CONTINUED OVER UNTIL THE MAIN ROTOR BLADES CONTACTED THE GROUND. THE HELICOPTER RECEIVED SUBSTANTIAL DAMAGE AND THE PILOT AND PASSENGER EGRESSED THROUGH THE BROKEN WINDSHIELD. THE FEMALE PASSENGER WAS TAKEN TO A LOCAL HOSPITAL AS A PRECAUTION AND LATER RELEASED. THE PILOT RECEIVED ONLY A MINOR SCRATCH ON HIS CHIN.

## The NTSB's investigation of this event (1)

From the NTSB's public API. Aircraft are matched to registry aircraft by N-number and event date, checked against the reported serial number, make and model; the registry's history has gaps, so older matches can be wrong. Compare the reported make and model.

- [NTSB CHI07CA094](https://openairdocs.org/ntsb/CHI07CA094), Mar 26, 2007: McCall Rotorway 162F (N162MC), Malta, IL, substantial damage. Probable cause: The pilot's improper use of the rotorcraft flight controls resulting in a dynamic rollover during takeoff. A factor was the soft ground that the left skid encountered.

From the FAA Accident/Incident Data System. Reports are matched to registry aircraft by N-number and event date, checked against the reported make and model; the registry's history has gaps, so older matches can be wrong. Compare the reported make and model. For accidents, the NTSB's own investigation is the fuller record.
