## Narrative

As the FAA filed it, in capitals; the FAA replaces private details with ^PRIVACY DATA OMITTED^.

(-23) INCLUDING THE REPORTING INSPECTOR, FOUR(4) HNL-ASI, 2 OPPS, & 2 A/W, WERE ASSIGNED TO THIS ON SITE INVESTIGATION, APPROXIMATELY 20 MAN HOURS WERE DEDICATED TO ON SITE ON SCENE. THE AIRCRAFT ENCOUNTERED A COLLISION WITH THE EARTH UPON ATTEMPTING EMERGENCY AUTOROTATION PROCEDURES AT A LOCALLY OWNED HELIPORT PAD ON THE NORTH SHORE HOTEL FACILITY ON THE ISLAND OF OAHU, AFTER THE AIRCRAFT EXPERIENCED A LOSS OF ENGINE POWER AS REPORTED BY THE PILOT. PRIOR TO THE LOSS OF POWER, AN ENGINE CHIP-DETECTOR LIGHT ILLUMINATED AND A TOT INCREASE WAS NOTICED, THE PILOT'S STATEMENTS MAKE REFERENCE THAT THE ENGINE MALFUNCTIONS BEGAN TO OCCUR WHILE THE AIRCRAFT WAS AT SLOW CRUISE FLIGHT AT APPROXIMATELY 300 FEET AGL. THE FLIGHT HAD BEEN ENROUTE FOR A 1.5 HOUR DURATION. FLIGHT WAS DISPATCHED FOR A LOCAL AREA DRUG ERADICATION SURVEILLANCE MISSION UNDER 14 CFR PART 91. ENGINE FAILURE MALFUNCTION INDICATIONS OCCURRED APPROXIMATELY 1.0 MILES FROM THE PILOT'S CHOSEN INTENDED LANDING SITE. THE AIRCRAFT SUSTAINED SUBSTANTIAL MAJOR AIRFRAME AND ROTOR DAMAGE, UPON COMPLETION OF ATTEMPTED AUTOROTATION FLARE. THE PRELIMINARY POST ACCIDENT INVESTIGATION ON-SITE SHOWED METAL SHAVINGS ON THE UPPER CHIP DETECTOR PLUG.

## The NTSB's investigation of this event (1)

From the NTSB's public API. Aircraft are matched to registry aircraft by N-number and event date, checked against the reported serial number, make and model; the registry's history has gaps, so older matches can be wrong. Compare the reported make and model.

- [NTSB SEA07LA108](https://openairdocs.org/ntsb/SEA07LA108), Apr 20, 2007: Hughes 369D (N911FT), Kahuku, HI, substantial damage. Probable cause: Fatigue cracking of the oil line from the engine gearbox to the oil pressure gauge, which resulted in loss of engine lubricating oil, leading to seizure of the power turbine.

From the FAA Accident/Incident Data System. Reports are matched to registry aircraft by N-number and event date, checked against the reported make and model; the registry's history has gaps, so older matches can be wrong. Compare the reported make and model. For accidents, the NTSB's own investigation is the fuller record.
