## Narrative

As the FAA filed it, in capitals; the FAA replaces private details with ^PRIVACY DATA OMITTED^.

(-23) SEQUENCE: THE ACCIDENT AIRCRAFT, N6511R LANDED AT TONOPAH AIRPORT, NV (KTPH) AND WAS PROVIDED SERVICE FROM AN AIRPORT FBO. AT 1933 UTC, N6511R WAS SERVICED WITH AN ADDITIONAL 10 GALLONS OF FUEL WHICH PLACED THE AIRPLANE AT ITS MAXIMUM GROSS WEIGHT FOR TAKEOFF. AT APPROXIMATELY 2100 UTC, N6511R TAXIED TO THE BEGINNING OF RUNWAY 33 AT KTPH. THE AIRPORT ASOS AT 2143Z INDICATED WINDS FROM 280 AT 15 KNOTS GUSTING TO 25 KNOTS WITH 10 MILES VISIBILITY, TEMPERATURE OF 31 DEGREES CELSIUS, AND DENSITY ALTITUDE OF 8300' MSL. N6511R DEPARTED RUNWAY 33 AND EXPERIENCED AN UNCOMMANDED 30 DEGREE ROLL TO THE RIGHT AND 20 DEGREE NOSE DOWN PITCH APPROXIMATELY 6000' FROM THE BEGINNING OF RUNWAY 33 AT AN ALTITUDE OF 30' AGL. THE UNCOMMANDED ROLL AND PITCH OCCURRED IMMEDIATELY AFTER GEAR RETRACTION WHEN THE AIRCRAFT'S LEFT FRONT DOOR UNEXPECTEDLY POPPED OPEN. THERE WAS NO REPORTED AIR TRAFFIC PRIOR TO TAKEOFF. THE AIRCRAFT IMPACTED THE GROUND 300' TO THE RIGHT OF THE EDGE OF RUNWAY 33 ON A TRAJECTORY OF 010 DEGREES. THE INITIAL POINT OF GROUND CONTACT WAS APPROXIMATELY 45 YARDS FROM THE FINAL RESTING POINT OF THE AIRCRAFT AND WAS IN THE FORM OF A GROOVE IN THE GROUND APPROXIMATELY 15 FEET LONG AND THEN STOPPED. THE RIGHT WING TIP APPEARS TO HAVE CREATED THE GROUND SCARRING. A SECOND AND LARGER POINT OF IMPACT WAS APPROXIMATELY 10 FEET FROM THE END OF THE INITIAL POINT OF CONTACT AND APPEARS TO BE WHERE THE ENTIRE AIRCRAFT IMPACTED THE GROUND. THE GROUND SCARRING CONTINUED FOR APPROXIMATELY ANOTHER 37 YARDS AND IS WHERE THE AIRCRAFT CAME TO REST AT A LEVEL ATTITUDE WITH A HEADING OF 060 DEGREES.

## The NTSB's investigation of this event (1)

From the NTSB's public API. Aircraft are matched to registry aircraft by N-number and event date, checked against the reported serial number, make and model; the registry's history has gaps, so older matches can be wrong. Compare the reported make and model.

- [NTSB SEA07LA163](https://openairdocs.org/ntsb/SEA07LA163), Jun 17, 2007: Beech B24R (N6511R), Tonopah, NV, substantial damage. Probable cause: The pilot's failure to maintain airspeed, which resulted in an inadvertent stall during the takeoff initial climb. The pilot's diverted attention to an open door, high density altitude, and wind…

From the FAA Accident/Incident Data System. Reports are matched to registry aircraft by N-number and event date, checked against the reported make and model; the registry's history has gaps, so older matches can be wrong. Compare the reported make and model. For accidents, the NTSB's own investigation is the fuller record.
