## Narrative

As the FAA filed it, in capitals; the FAA replaces private details with ^PRIVACY DATA OMITTED^.

(-23) AT APPROXIMATELY 1147 HOURS LOCAL TIME THE PILOT ^PRIVACY DATA OMITTED^ AND ONE PASSENGER ATTEMPTED TO DEPART LAKE JOHN, NEAR LAKEWOOD, WI. IN N914JS, AN EXPERIMENTAL/AMATEUR BUILT, AIRVENTURA II. WITNESSES STATED THE AMPHIBIAN AIRCRAFT CIRCLED THE LAKE TWICE BEFORE ATTEMPTING TO DEPART TO THE SOUTHWEST. THE AIRCRAFT BECAME AIRBORNE AND THEN CLIMBED STEEPLY TO CLEAR TREES, LEVELING OFF BRIEFLY BEFORE THE LEFT WING DROPPED AND THE AIRCRAFT FELL NOSE FIRST INTO THE GROUND. FIRST RESPONDERS REPORT THE AIRCRAFT ENGINE CONTINUED TO OPERATE AT FULL THROTTLE FOR TEN TO FIFTEEN MINUTES AFTER THE CRASH AS NO ONE KNEW HOW TO SHUT IT DOWN. THE PILOT SURVIVED THE CRASH BUT DIED OF HIS INJURIES LATER THAT NIGHT. THE PASSENGER SUSTAINED SERIOUS INJURY BUT IS EXPECTED TO SURVIVE.

## The NTSB's investigation of this event (1)

From the NTSB's public API. Aircraft are matched to registry aircraft by N-number and event date, checked against the reported serial number, make and model; the registry's history has gaps, so older matches can be wrong. Compare the reported make and model.

- [NTSB CHI07LA238](https://openairdocs.org/ntsb/CHI07LA238), Jul 28, 2007: Swanson Aventura II (N914JS), Lakewood, WI, substantial damage, 1 fatal. Probable cause: The pilot's excessive pull up, his not maintaining airspeed, and the inadvertent stall resulting in the loss of control. A factor contributing to the accident was the trees.

From the FAA Accident/Incident Data System. Reports are matched to registry aircraft by N-number and event date, checked against the reported make and model; the registry's history has gaps, so older matches can be wrong. Compare the reported make and model. For accidents, the NTSB's own investigation is the fuller record.
