## Narrative

As the FAA filed it, in capitals; the FAA replaces private details with ^PRIVACY DATA OMITTED^.

(-23) INVESTIGATION INDICATED AIRCRAFT IMPACTED WITH TERRAIN DUE TO LOSS OF CONTROL. WITNESS INDICATED THAT THE AIRCRAFT DESCENDED AT AN APPROXIMATELY 60 DEGREE ANGLE AT A HIGH RATE OF SPEED. WITNESS STATED THAT HE HEARD A LOUD BANG JUST BEFORE IMPACT AND SAW SOMETHING BLACK FLY OFF. WITNESS SAID THE ROTOR APPEARED TO BE TURNING SLOWLY PRIOR TO THE CRASH. CONING OF ROTOR WAS DISCOVERED AT THE CRASH SITE. POST CRASH INSPECTION OF THE WRECKAGE INDICATED CONTROL CONTINUITY AND THAT THE ENGINE WAS MAKING POWER PRI0R TO IMPACT. HOWEVER, ONE OF THREE PROPELLER BLADES WAS NOT FOUND WHICH MAY HAVE BEEN HIT BY THE ROTOR. IMPACT MARKS ON THE RUDDER INDICATED THAT THE ROTOR HAD STRUCK THE RUDDER.

## The NTSB's investigation of this event (1)

From the NTSB's public API. Aircraft are matched to registry aircraft by N-number and event date, checked against the reported serial number, make and model; the registry's history has gaps, so older matches can be wrong. Compare the reported make and model.

- [NTSB DFW08LA052](https://openairdocs.org/ntsb/DFW08LA052), Dec 29, 2007: Althouse RAF 2000 GTX (N136DG), Crowley, TX, destroyed, 2 fatal. Probable cause: The pilot's failure to maintain rotor rpm while maneuvering, which resulted in a loss of rotor blade rigidity.

From the FAA Accident/Incident Data System. Reports are matched to registry aircraft by N-number and event date, checked against the reported make and model; the registry's history has gaps, so older matches can be wrong. Compare the reported make and model. For accidents, the NTSB's own investigation is the fuller record.
