As the FAA filed it, in capitals; the FAA replaces private details with ^PRIVACY DATA OMITTED^.

(-23) AT 0113Z, NWA 1203 AT FL320 REQUESTED A DESTINATION DIVERSION TO FWA OR CLOSEST AIRPORT DUE TO OXYGEN TANK FAILURE. ZAU ATC ACKNOWLEDGED NW 1203'S REQUEST AND CLEARED NWA 1203 TO FWA AND ASSIGNED A 280 HEADING FOR THE DESCENT DUE TO OPPOSITE DIRECTION TRAFFIC. NWA 1203 WAS ASKED IF THEY WERE DECLARING AN EMERGENCY. NWA 1203 RESPONDED "AFFIRMATIVE." AREA FLM WAS IMMEDIATELY NOTIFIED AND ALL PERTINENT DETAILS WERE COORDINATED WITH THE ZAU OLK SECTOR. ONCE NWA 1203 VACATED FL300 THE AIRCRAFT WAS TURNED TO A HEADING OF 180 AND WAS GIVEN THE FWA VOR FREQUENCY. ZAU OLK SECTOR COORDINATED A DESCENT TO 11,000' AND NWA 1203 WAS CLEARED TO 11,000' AND GIVEN THE LOCAL ALTIMETER SETTING. COMMUNICATIONS WERE TRANSFERRED TO ZAU OLK SECTOR AT 0116Z. NWA 1203 CHECKED ON FREQUENCY 119.85 DESCENDING TO 11,000'. NWA WAS ASKED IF EQUIPMENT WOULD BE NECESSARY AND THE NUMBER OF PEOPLE ON BOARD. THE INFORMATION WAS COORDINATED WITH THE SUPERVISOR AND THE AIRCRAFT SWITCHED TO FWA APPROACH CONTROL.

From the FAA Accident/Incident Data System. Reports are matched to registry aircraft by N-number and event date, checked against the reported make and model; the registry's history has gaps, so older matches can be wrong. Compare the reported make and model. For accidents, the NTSB's own investigation is the fuller record.
