## Narrative

As the FAA filed it, in capitals; the FAA replaces private details with ^PRIVACY DATA OMITTED^.

(-23) THIS AIRPLANE LOST POWER AFTER DEPARTING BIH RUNWAY 12. THE PILOT WAS ABLE TO RETURN TO RUNWAY 12 BUT WITNESSES SAY HE APPROACHED TOO HIGH, TOO FAST AND HAD TO INTENTIONALLY GROUND LOOP IN THE OVERRUN AREA TO PRECLUDE RUNNING THROUGH THE AIRPORT BOUNDARY FENCE. NO INJURIES TO THREE SOULS ONBOARD. ELT DID NOT ACTIVATE AND THERE WERE NO FIRES. RIGHT LANDING GEAR IS COLLAPSED AND OUTBOARD PORTION OF RIGHT ELEVATOR IS BENT DOWN. RIGHT WING SPAR AND SKIN IS BENT UP AND WRINKLED FROM COMPRESSION LOAD WHEN THE WING TIP IMPACTED THE GROUND. PHOTOS ARE AVAILABLE. POST ACCIDENT INVESTIGATION DISCOVERED THAT THE AUTOMATIC ENGINE ALTERNATE INDUCTION AIR VALVE CAME APART AND SOME OF THE SMALL PARTS (TWO INCH AN3 BOLT, TWO INCH SPRING, A COTTER PIN, AND A WASHER) WERE INGESTED INTO THE ENGINE INTAKE MANIFOLD. THE AN3 (ONE QUARTER INCH DIAMETER) BOLT WAS FOUND LODGED IN (HOLDING OPEN) THE NUMBER SIX ENGINE CYLINDER EXHAUST VALVE. THIS CONDITION RESULTED IN A MAJOR LOSS OF ENGINE POWER, ESPECIALLY WHEN THE PARTS WENT THROUGH THE INTAKE VALVES. THE SPIRAL SPRING, WASHER, AND COTTER PIN WERE NOT FOUND; EVIDENTLY THEY PASSED THORUGH THE ENGINE AND OUT THE TAIL PIPE.

## The NTSB's investigation of this event (1)

From the NTSB's public API. Aircraft are matched to registry aircraft by N-number and event date, checked against the reported serial number, make and model; the registry's history has gaps, so older matches can be wrong. Compare the reported make and model.

- [NTSB SEA08LA087](https://openairdocs.org/ntsb/SEA08LA087), Mar 7, 2008: Maule M-4 (N51430), Bishop, CA, substantial damage. Probable cause: A loss of engine power due to foreign object ingestion in the induction system, which resulted in a stuck/jammed exhaust valve during the takeoff.

From the FAA Accident/Incident Data System. Reports are matched to registry aircraft by N-number and event date, checked against the reported make and model; the registry's history has gaps, so older matches can be wrong. Compare the reported make and model. For accidents, the NTSB's own investigation is the fuller record.
