## Narrative

As the FAA filed it, in capitals; the FAA replaces private details with ^PRIVACY DATA OMITTED^.

(-23) THE PILOT WAS TAXIING OUT FOR TAKEOFF AND IN THE NINETY DEGREE TURN ONTO THE RUNWAY HE SAID HE HEARD A BANG AND THE AIRPLANE SEEMED TO HOP AT THE SAME TIME. HE SAID THE AIRPLANE BEGAN TO SETTLE, HE SHUT OFF THE ENGINES, BUT THE RIGHT PROPELLER TIPS STRUCK THE RUNWAY. THE NOSE GEAR REMAINED DOWN, BOTH MAIN GEAR PARTIALLY RETRACTED, AND THE BELLY SLID ON THE RUNWAY AND SUSTAINED SUBSTANTIAL DAMAGES. DURING POST ACCIDENT INVESTIGATION WE DISCOVERED THAT THE CLEVIS ON THE INBOARD HYDRAULIC LANDING GEAR ACTUATOR ON THE LEFT MAIN LANDING GEAR IS BROKEN AND THE ACTUATOR IS DISCONNECTED FROM THE LANDING GEAR HOUSING. FURTHER INVESTIGATION FOUND THAT THE CLEVIS WAS INCORRECTLY INSTALLED. THE BOLT THAT SECURES THE CLEVIS INTO THE LANDING GEAR HOUSING WAS NOT SCREWED INTO THE BARREL NUT, BUT WEDGED IN BETWEEN THE EDGE OF THE HOLE IN THE HOUSING, THE EDGE OF THE HOLE IN THE CLEVIS, AND THE END OF THE BARREL NUT. APPARENTLY THE BARREL NUT SLID OUT OF PLACE DURING ASSEMBLY. IT HAS BEEN ONE MONTH AND 14.4 OPERATING HOURS SINCE A CERTIFIED REPAIR STATION PERFORMED A COMPLETE INSPECTION OF THIS AIRPLANE, INCLUDING REMOVING AND REPLACING THE CLEVIS FOR NON-DESTRUCTIVE TESTING.

## The NTSB's investigation of this event (1)

From the NTSB's public API. Aircraft are matched to registry aircraft by N-number and event date, checked against the reported serial number, make and model; the registry's history has gaps, so older matches can be wrong. Compare the reported make and model.

- [NTSB SEA08LA117](https://openairdocs.org/ntsb/SEA08LA117), May 3, 2008: Rockwell 690C (N980AK), Minden, NV, substantial damage. Probable cause: The collapse of both main landing gear for undetermined reasons.

From the FAA Accident/Incident Data System. Reports are matched to registry aircraft by N-number and event date, checked against the reported make and model; the registry's history has gaps, so older matches can be wrong. Compare the reported make and model. For accidents, the NTSB's own investigation is the fuller record.
