## Narrative

As the FAA filed it, in capitals; the FAA replaces private details with ^PRIVACY DATA OMITTED^.

(-23) ^PRIVACY DATA OMITTED^, AFFILIATED AS AN INTSTRUCTOR WITH SKYDIVE ELSINORE AND A USPA D LICENSE, PRO RATED PARACHUTIST WITH OVER 4,000 JUMPS, WAS FATALLY INJURED AS A RESULT OF HIS RESERVE CHUTE NOT BEING DEPLOYED IN SUFFICIENT TIEM TO AVOID IMPACTIN THE SURFACE. HIS MAIN CANOPY CONTROL TOGGLE WAS COMPROMISED BY A TENSION KNOT. HIS HARNESS WAS FOUND TO BE LOOSENED, WHICH CAUSED REPOSITIONING OF THE RESERVE CHUTE ACTIVATION MECHANISM, WHICH WAS FOUND STILL STOWED IN THE VELCRO POCKET. AN AUTOMATIC RESERVE CHUTE DEPLOYMENT SYSTEM WAS NOT INSTALLED ON THE RESERVE CHUTE.

From the FAA Accident/Incident Data System. Reports are matched to registry aircraft by N-number and event date, checked against the reported make and model; the registry's history has gaps, so older matches can be wrong. Compare the reported make and model. For accidents, the NTSB's own investigation is the fuller record.
