## Narrative

As the FAA filed it, in capitals; the FAA replaces private details with ^PRIVACY DATA OMITTED^.

DURING A COMMERCIAL AMEL PRACTICAL TEST, THE APPLICANT AND PILOT EXAMINER FAILED TO RECOGNIZE A “DOOR OPEN” WARNING INDICATION ON THE GARMIN 1000 DISPLAY PRIOR TO TAKEOFF. DURING TAKEOFF ROLL, THE REAR PASSENGER DOOR BLEW OPEN, RIPPED AWAY FROM ITS HINGES, AND DEPARTED THE AIRFRAME. THE DOOR DID NOT STRIKE ANY PORTION OF THE AIRFRAME, AND DAMAGE WAS LIMITED TO THE DOOR HINGES AND DOOR ASSEMBLY. THE PILOTS ABORTED THE TAKEOFF WITHOUT FURTHER INCIDENT.

From the FAA Accident/Incident Data System. Reports are matched to registry aircraft by N-number and event date, checked against the reported make and model; the registry's history has gaps, so older matches can be wrong. Compare the reported make and model. For accidents, the NTSB's own investigation is the fuller record.
