## Narrative

As the FAA filed it, in capitals; the FAA replaces private details with ^PRIVACY DATA OMITTED^.

(-23) THE AIR CANADA A-319 REJECTED THE TAKEOFF ON RWY 25 BECAUSE THE PILOT'S SLIDING WINDOW WAS NOT CLOSED AND LOCKED. AFTER THE REJECTED TAKEOFF THE CREW REQUESTED TAXI TO AN AREA WHERE THE AIRCRAFT COULD BE PARKED AND ENGINES SHUT DOWN FOR ABOUT 45 MINUTES. THE TIME BETWEEN THE REJECTED TAKEOFF AND FOLLOWING TAKEOFF WAS ABOUT 1 HOUR. AFTER THE SECOND TAKEOFF THE FLAPS DID NOT RETURN TO THE FULL UP POSITION. AN EMERGENCY WAS DECLARED AND THE AIRCRAFT RETURNED FOR A LANDING ON RWY 16R. WHILE CONFIGURING FOR THE LANDING A GEAR NOT DOWN WARNING WAS DETECTED. ALL APPLICABLE PROCEDURES WERE COMPLETED AND THE AIRCRAT LANDED WITHOUT INCIDENT ON RWY 25. THE CREW STOPPED THE AIRCRAFT ON THE RUNWAY, THE PASSENGERS WERE DISEMBARKED AND TRANSPORTED TO THE TERMINAL. THE AIRCRAFT WAS TOWED TO PARKING. INSPECTION REVEALED THAT THE RIGHT INBOARD TIRE HAD SHREDDED. THE TIRE COULD HAVE FAILED ON TAKEOFF OR SOMETIME AFTER LIFT OFF. THERE WERE TWO DENTS ON THE RIGHT FLAP, PLUS A DENT ON THE WING TO BODY FAIRING FORWARD OF THE AFT CARGO DOOR. MAINTENANCE FOUND THE GEAR BOX PROX SENSOR WAS MISSING.

From the FAA Accident/Incident Data System. Reports are matched to registry aircraft by N-number and event date, checked against the reported make and model; the registry's history has gaps, so older matches can be wrong. Compare the reported make and model. For accidents, the NTSB's own investigation is the fuller record.
