## Narrative

As the FAA filed it, in capitals; the FAA replaces private details with ^PRIVACY DATA OMITTED^.

(-23) WHILE IN CRUISE FLIGHT THE AIRCRAFT EXPERIENCED A LOSS OF POWER AND SUBSEQUENT FLAMEOUT OF THE ENGINE. PILOT INITIATED AN AUTO ROTATION INTO A CANYON. THE AIRCRAFT CAME TO REST IN A HEAVILY WOODED AREA WITH SEVERE DAMAGE TO THE AIRFRAME INCLUDING TAILBOOM, TAIL ROTOR, AND STABILIZERS. AIRCRAFT ROLLED OVER ONTO TO ITS SIDE. THE PILOT ESCAPED WITHOUT INJURY AND WALKED OUT TO CONTACT HELP. FOLLOWING DAY AIRCRAFT WAS RECOVERED AND TRAILERED TO A HANGER. DURING EXAMINATION OF THE ENGINE, A FUEL DRAIN CAP WAS FOUND LOOSE AND NO FUEL WAS FOUND IN THE FUEL SUPPLY LINES TO THE FUEL NOZZLE. ENGINE IS BEING SENT TO INDIANAPOLIS TEST FACILITY FOR RUNS AND FURTHER EXAMINATION.

## The NTSB's investigation of this event (1)

From the NTSB's public API. Aircraft are matched to registry aircraft by N-number and event date, checked against the reported serial number, make and model; the registry's history has gaps, so older matches can be wrong. Compare the reported make and model.

- [NTSB WPR09LA048](https://openairdocs.org/ntsb/WPR09LA048), Nov 25, 2008: Hughes 369HS (N9154F), Fillmore, UT, substantial damage. Probable cause: The loss of engine power due to fuel starvation as a result of a loose fuel filter upper drain cap, which was not properly secured by maintenance personnel.

From the FAA Accident/Incident Data System. Reports are matched to registry aircraft by N-number and event date, checked against the reported make and model; the registry's history has gaps, so older matches can be wrong. Compare the reported make and model. For accidents, the NTSB's own investigation is the fuller record.
