## Narrative

As the FAA filed it, in capitals; the FAA replaces private details with ^PRIVACY DATA OMITTED^.

(-23) ^PRIVACY DATA OMITTED^ WAS THE PILOT IN COMMAND OF N833SP WHEN IT IMPACTED THE BERM OF AN INTERSECTING TAXIWAY OF RUNWAY 28 AT WAUKESHA AIRPORT. ^PRIVACY DATA OMITTED^ NOTICED DURING THE TAXI OUT THAT THE AUTOPILOT WAS BLINKING AND MAKING WARNING TONES WHILE TAXIING TO RUNWAY 28. AFTER NOT BEING ABLE TO RETURN THE AUTOPILOT TO THE STANDBY MODE BY CYCLING THE AP BUTTON, ^PRIVACY DATA OMITTED^ CYCLED THE AVIONICS MASTER SWITCH. AFTER THE AUTOPILOT POWERED UP AND DID A SELF CHECK IT WENT TO THE NORMAL STANDBY MODE. ON THE TAKEOFF ROLL THE AIRCRAFT "JUMPED" INTO THE AIR AT A LOW SPEED AND FELT "WRONG" ON THE CONTROLS. ^PRIVACY DATA OMITTED^ ABORTED THE TAKEOFF AND TOUCHED BACK DOWN OFF THE LEFT SIDE OF THE RUNWAY. THE AIRCRAFT PROCEEDED TO HIT THE FIRST OF TWO SNOW BANKS AND AGAIN BECAME AIRBORNE. AFTER SETTLING BACK DOWN THE AIRCRAFT HIT THE SECOND SNOW BANK AND BECAME AIRBORNE AGAIN THIS TIME TOUCHING DOWN NOSE FIRST AGAINST THE DIRT BERM OF AN INTERESTING TAXIWAY. THIS CAUSED THE NOSEWHEEL TO SEPARATE FROM THE AIRCRAFT AND THE RIGHT MAIN GEAR TO FAIL. THE TRIM WHEEL WAS NOTED 3/4 WAY NOSE UP AT THE ACCIDENT SITE.
