## Narrative

As the FAA filed it, in capitals; the FAA replaces private details with ^PRIVACY DATA OMITTED^.

ON JANUARY 22, 2009 HYANNIS AIR SERVICE, DBA CAPE AIR (HYIA), FLIGHT 9399, REPORTED BOTH ENGINES LOST POWER WHILE ENROUTE FROM KEY WEST, FL (EYW) TO FT. MYERS, FL (RSW). APPROXIMATELY 60 NAUTICAL MILES SOUTH OF RSW THE PILOT NOTICED AN FUEL IMBALANCE OF 300 POUNDS IN THE LEFT TANK AND 50 POUNDS IN THE RIGHT TANK. THE PILOT ATTEMPTED TO CORRECT THE IMBALANCE BY CROSS FEEDING FUEL FROM THE LEFT TANK. AT 4000 FEET JUST NORTH APF, THE PILOT REPORTED THAT THE RIGHT ENGINE BEGAN TO SURGE AND THE LEFT ENGINE BEGAN TO SURGE. THE PILOT DECLARED AN EMERGENCY, AT APPROXIMATELY 3000 FEET, TURNED 180 DEGREES BACK TO APF. THE PILOT FEATHERED BOTH ENGINES AND GLIDED INTO APF, LANDING WITH NO INJURIES TO PERSONS ON BOARD AND NO DAMAGE TO THE AIRCRAFT.

## The NTSB's investigation of this event (1)

From the NTSB's public API. Aircraft are matched to registry aircraft by N-number and event date, checked against the reported serial number, make and model; the registry's history has gaps, so older matches can be wrong. Compare the reported make and model.

- [NTSB ERA09IA140](https://openairdocs.org/ntsb/ERA09IA140), Jan 23, 2009: Cessna 402C (N2615G), Naples, FL, incident, no damage. Probable cause: The pilot's failure to recognize that both engines were being supplied fuel only from the right main fuel tank, resulting in fuel starvation and a subsequent loss of engine power from both engines.…
