## Narrative

As the FAA filed it, in capitals; the FAA replaces private details with ^PRIVACY DATA OMITTED^.

AT 1045 CST, AIRCRAFT IMPACTED THE GROUND APPROXIMATELY 6 MILES WEST OF KDWH, CFI STATES THAT THE STUDENT PILOT WAS CONDUCTING A 180 DEGREE PRACTICE AUTO ROTATION FROM 700 FEET. AFTER STUDENT PILOT LOST CONTROL AROUND 250 FEET, CFI TOOK CONTROL OF AIRCRAFT BUT WAS UNABLE TO RECOVER PRIOR TO IMPACTING THE GROUND. AIRCRAFT IMPACTED THE GROUND TAIL FIRST CAUSING THE TAIL ROTOR BLADES, TAIL DRIVE SHAFT, AND TAIL SECTION SUBSTANTIAL DAMAGE. THE AIRCRAFT SLID ON ITS SKIDS FOR ANOTHER 40-50 FEET, BEFORE HITTING A LOW, UNEVEN SPOT IN THE GROUND. ## The NTSB's investigation of this event (1)

From the NTSB's public API. Aircraft are matched to registry aircraft by N-number and event date, checked against the reported serial number, make and model; the registry's history has gaps, so older matches can be wrong. Compare the reported make and model.

- [NTSB CEN09CA157](https://openairdocs.org/ntsb/CEN09CA157), Feb 6, 2009: Robinson Helicopter R22 Beta (N234LA), Cypress, TX, substantial damage. Probable cause: The student pilot's failure to maintain adequate main rotor rpm and the instructor's delayed remedial action.

From the FAA Accident/Incident Data System. Reports are matched to registry aircraft by N-number and event date, checked against the reported make and model; the registry's history has gaps, so older matches can be wrong. Compare the reported make and model. For accidents, the NTSB's own investigation is the fuller record.
