## Narrative

As the FAA filed it, in capitals; the FAA replaces private details with ^PRIVACY DATA OMITTED^.

(-23) THE CONTROL CABLE CONTINUITY WAS FOUND TO BE COMPLETE AT THE ACCIDENT SCENE. THE PILOT IN COMMAND INDICATED THAT THE CONTROLS WERE FREE AND MOVEABLE, AND THERE WAS NO INDICATION OF ANY ENGINE SURGE OR ENGINE ABNORMALITY DURING THE POST-CRASH INTERVIEWS. THE AIRWORTHINESS INSPECTOR WHO RESPONDED AT THE SCENE FOUND EVIDENCE THAT THE RIGHT WING OF THE AIRCRAFT STRUCK THE RUNWAY SURFACE. IN THE IIC'S OPINION, THE INSTRUCTOR DID NOT PROPERLY SUPERVISE THE UNCERTIFICATED PILOT DURING DUAL INSTRUCTION AND ALLOWED A LOSS OF CONTROL TO DEVELOP DURING A SIMULATED SOFT-FIELD TAKEOFF. DURING THE ATTEMPTED RECOVERY, THERE WAS NOT A CLEAR UNDERSTANDING OF WHO HAD CONTROL OF THE AIRCRAFT.

## The NTSB's investigation of this event (1)

From the NTSB's public API. Aircraft are matched to registry aircraft by N-number and event date, checked against the reported serial number, make and model; the registry's history has gaps, so older matches can be wrong. Compare the reported make and model.

- [NTSB ERA09LA189](https://openairdocs.org/ntsb/ERA09LA189), Mar 7, 2009: Cirrus Design SR22 (N574PG), Montauk, NY, substantial damage. Probable cause: The student pilot's failure to maintain directional control during takeoff. Contributing to the accident was the instructor's delayed remedial action.

From the FAA Accident/Incident Data System. Reports are matched to registry aircraft by N-number and event date, checked against the reported make and model; the registry's history has gaps, so older matches can be wrong. Compare the reported make and model. For accidents, the NTSB's own investigation is the fuller record.
