- [NTSB WPR09LA202](https://openairdocs.org/ntsb/WPR09LA202), Apr 18, 2009: Alexander Thomas P Sportsman GS-2 (N209TP), El Dorado HIlls, CA, destroyed, 1 fatal. Probable cause: The in-flight separation of the elevator control torque tube rod end bearing from the elevator torque tube as a result of incorrectly tightened rod end bearing lock nuts.

From the FAA Accident/Incident Data System. Reports are matched to registry aircraft by N-number and event date, checked against the reported make and model; the registry's history has gaps, so older matches can be wrong.
