## Narrative

As the FAA filed it, in capitals; the FAA replaces private details with ^PRIVACY DATA OMITTED^.

(-23) ^PRIVACY DATA OMITTED^ AN AIRLINE TRANSPORT RATED PILOT DEPARTED PIERCE COUNTY AIRPORT THUN FIELD AT APPROXIMATELY 1515 LCLC ON 05/01/2009 IN THE PILOTS CESSNA 182, WHEN THE AIRCRAFT LOST POWER. THIS WAS THE AIRCRAFT'S FIRST FLIGHT AFTER AN ANNUAL INSPECTION WAS PERFORMED, THE TANKS HAD BEEN DRAINED AND THE INNER CONNECT HOSES REPLACED. AFTER LANDING THE AIRCRAFT FLIPPED OVER, A SMALL DRIP WAS NOTICED COMING FROM THE AIRCRAFT. UPON INVESTIGATION, IT WAS DETERMINED THATA A MECHANIC HAD REFUELED THE AIRCRAFT OUT OF THE SAME 55 GALLON DRUM THE FUEL HAD BEEN DRAINED INTO. THE MECHANIC/IA STATED THAT HE HAD DRAINED 40 GALLONS FROM THE AIRCRAFT AND REFUELED THE AIRCRAFT WITH 20 GALLONS A SIDE. UPON INSPECTOR ARRIVAL, 25 GALLONS WAS DRAINED FROM THE RIGHT TANK AND THE LEFT TANK CONTAINED NO FUEL. IN A PHONE CONVERSATION WITH THE PILOT, HE STATED THAT DUE TO HEALTH PROBLEMS HE IS UNABLE TO VISUALLY VERIFY THE FUEL QUANTITY INSIDE THE TANKS AND THE FUEL TOTALIZER INDICATED THE SAME AS IT DID WHEN HE HAD LAST FLOWN THE AIRCRAFT.

## The NTSB's investigation of this event (1)

From the NTSB's public API. Aircraft are matched to registry aircraft by N-number and event date, checked against the reported serial number, make and model; the registry's history has gaps, so older matches can be wrong. Compare the reported make and model.

- [NTSB WPR09LA223](https://openairdocs.org/ntsb/WPR09LA223), May 1, 2009: Cessna 182Q (N382CH), Puyallup, WA, substantial damage. Probable cause: A loss of engine power due to fuel starvation as a result of the pilot’s failure to verify the fuel levels in the tanks, and, his failure to select the proper fuel tank.

From the FAA Accident/Incident Data System. Reports are matched to registry aircraft by N-number and event date, checked against the reported make and model; the registry's history has gaps, so older matches can be wrong. Compare the reported make and model. For accidents, the NTSB's own investigation is the fuller record.
