## Narrative

As the FAA filed it, in capitals; the FAA replaces private details with ^PRIVACY DATA OMITTED^.

(-23) ON 05/04/2009 ^PRIVACY DATA OMITTED^ WAS PILOT OF N17TD AN H-201B GLIDER THAT CRASHED ON TAKEOFF AND SUBSEQUENT LANDING FOLLOWING A WINCH LAUNCH AT MEADOWLAKE AIRPORT, COLORADO SPRINGS CO. THE WINCH FAILED TO ACCELERATE THE GLIDER TO THE CORRECT FLYING AND CLIMBING SPEED. AFTER A SHORT CLIMB TO 20 FT AGL AND AGLIDE FROM THAT ALTITUDE, THE GLIDER DESCENDED AND THEN FLARED HIGH IN THE APPROACH SEQUENCE. THE GLIDER THEN IMPACTED THE GROUND DISINTEGRATING THE MAIN WHEEL AND TIRE AND BREAKING THE FUSELAGE BEHIND THE AFT MAIN GEAR BULKHEAD.

## The NTSB's investigation of this event (1)

From the NTSB's public API. Aircraft are matched to registry aircraft by N-number and event date, checked against the reported serial number, make and model; the registry's history has gaps, so older matches can be wrong. Compare the reported make and model.

- [NTSB CEN09LA278](https://openairdocs.org/ntsb/CEN09LA278), May 4, 2009: Glasflugel H201B (N17TD), Meadow Lake Airport, CO, substantial damage. Probable cause: A programming anomaly within the winch's computer aided launch control system, which incorrectly limited the maximum winch speed during launch of the glider.

From the FAA Accident/Incident Data System. Reports are matched to registry aircraft by N-number and event date, checked against the reported make and model; the registry's history has gaps, so older matches can be wrong. Compare the reported make and model. For accidents, the NTSB's own investigation is the fuller record.
