## Narrative

As the FAA filed it, in capitals; the FAA replaces private details with ^PRIVACY DATA OMITTED^.

(-23) ON MAY 9, 2009, AT 1317 CENTRAL STANDARD TIME, A VELOCITY, N45YV, WAS DESTROYED UPON IMPACT WITH TERRAIN APPROXIMATELY 2 MILES SE OF HOUSTON EXECUTIVE AIRPORT (TME). THE PRIVATE PILOT, THE SOLE OCCUPANT OF THE AIRPLANE WAS FATALLY INJURED. A FRIEND OF THE PILOT REPORTED THAT THE PILOT HAD JUST INSTALLED A NEW PROPELLER ON THE AIRCRAFT AND WAS CONDUCTING A MAINTENANCE TEST FLIGHT. THE AIRCRAFT WAS LAST SEEN IN THE TRAFFIC PATTERN AT HOUSTON EXECUTIVE AIRPORT (TME) AROUND 1315 LOCAL TIME. A WITNESS 1 MILE SOUTHWEST OF THE ACCIDENT SIGHT REPORTED SEEING THE AIRCRAFT FLYING NORTH AND THEN HEARD THE ENGINE QUIT. WHEN HE LOOKED BACK UP AGAIN HE SAID THE AIRCRAFT WAS INVERTED AND FALLING STRAIGHT DOWN. AN ONE SCENE INVESTIGATION REVEALED THAT THE AIRCRAFT IMPACTED THE GROUND INVERTED AND WAS NOT PRODUCING POWER ON IMPACT. TWO ENGINE MOUNTS WERE BROKEN AND THE OTHER TWO WERE BENT. IT WAS EVIDENT THAT FUEL WAS ONBOARD, BUT BOTH TANKS WERE BREACHED AND THE FUEL DRAINED ONTO THE GROUND. THE PIC HAD BOUGHT 27 GALLONS OF FUEL ON MAY 9TH AT TIME PRIOR TO DEPARTURE. ADDITIONALLY, ONE OF THE THREE PROPELLER BLADES SEPARATED FROM THE PROPELLER HUB AND WAS FOUND 400-600' WEST OF THE WRECKAGE.

## The NTSB's investigation of this event (1)

From the NTSB's public API. Aircraft are matched to registry aircraft by N-number and event date, checked against the reported serial number, make and model; the registry's history has gaps, so older matches can be wrong. Compare the reported make and model.

- [NTSB CEN09LA288](https://openairdocs.org/ntsb/CEN09LA288), May 9, 2009: Smysor Kevin Velocity (N45YV), Katy, TX, substantial damage, 1 fatal. Probable cause: The pilot's loss of control following an in-flight failure of the propeller.

From the FAA Accident/Incident Data System. Reports are matched to registry aircraft by N-number and event date, checked against the reported make and model; the registry's history has gaps, so older matches can be wrong. Compare the reported make and model. For accidents, the NTSB's own investigation is the fuller record.
