## Narrative

As the FAA filed it, in capitals; the FAA replaces private details with ^PRIVACY DATA OMITTED^.

(-23) THE PILOT STATED THAT HE WAS APPROACHING THE SOUTH SIDE OF TAXIWAY WHERE HE PLANNED TO HOVER AT 10 FEET AND FLY TO HIS HANGAR. DURING TRANSITION, HE LOST TAIL ROTOR EFFECTIVENESS, AND THE AIRCRAFT IMPACTED THE TAXIWAY, BOUNCED, AND SPUN. HE WAS UNSURE OF ALL THAT HAPPENED DURING THE SEQUENCE. THE AIRCRAFT LANDED UPRIGHT, WITH SUBSTANTIAL DAMAGE. THE TAIL ROTOR AND MAIN ROTOR BLADES IMPACTED THE GROUND. THE MAIN ROTOR BLADES IMPACTED THE TAIL BOOM. THE LANDING GEAR COLLAPSED AT THE LEFT REAR ATTACH POINT. THE FUSELAGE BENT BEHIND THE CABIN AREA. THE PILOT/BUILDER INDICATED THAT THE AIRCRAFT COULD BE REBUILT. THE TAIL ROTOR AND MAIN ROTOR BLADES INDICATE THAT THEY WERE NOT A FULL SPEED WHEN THE AIRCRAFT IMPACTED. FURTHER INFORMATION FROM THE PILOT INDICATED THAT HE WAS PERFORMING AN AUTOROTATION.

## The NTSB's investigation of this event (1)

From the NTSB's public API. Aircraft are matched to registry aircraft by N-number and event date, checked against the reported serial number, make and model; the registry's history has gaps, so older matches can be wrong. Compare the reported make and model.

- [NTSB CEN09CA394](https://openairdocs.org/ntsb/CEN09CA394), Jun 27, 2009: Sidler Mike Rotorway (N1934), Waxahachie, TX, substantial damage. Probable cause: The pilot’s failure to control his rate of descent during autorotation, which required an aggressive recovery maneuver and resulted in a sudden loss of tail rotor effectiveness.

From the FAA Accident/Incident Data System. Reports are matched to registry aircraft by N-number and event date, checked against the reported make and model; the registry's history has gaps, so older matches can be wrong. Compare the reported make and model. For accidents, the NTSB's own investigation is the fuller record.
