## Narrative

As the FAA filed it, in capitals; the FAA replaces private details with ^PRIVACY DATA OMITTED^.

(-23) PILOT DEPARTED AIRFILED TO THE NORTH WITH A KNOWN CRACK IN THE WOODEN PROPELLER. AT ABOUT 500 FEET, HE FELT A SEVERE SHAKING AND SAW THE PROPELLER DISINEGRATE. HE TURNED IN AN ATTEMPT TO RETURN TO THE GRASS RUNWAY. TO AVOID THE TRAFFIC ON INTERSTATE 10, IMMEDIATELY SOUTH OF THE AIRFIELD, HE CONTINUED CROSS THE RUNWAY AND LANDED IN AN AN ADJACENT FIELD. HE SAW A LEVEE, APPROXIMATELY 3 FEET HIGH BUT COULD NOT AVOID IT. HE HIT THE LEVEE AND THE AIRPLANE FLIPPED OVER.

## The NTSB's investigation of this event (1)

From the NTSB's public API. Aircraft are matched to registry aircraft by N-number and event date, checked against the reported serial number, make and model; the registry's history has gaps, so older matches can be wrong. Compare the reported make and model.

- [NTSB CEN09CA510](https://openairdocs.org/ntsb/CEN09CA510), Jul 4, 2009: Wohnoutka Steven M Team Z MAX (N301SS), Iowa, LA, substantial damage. Probable cause: A total loss of engine thrust during initial climb due to the failure of the airplane's propeller as a result of preexisting damage.

From the FAA Accident/Incident Data System. Reports are matched to registry aircraft by N-number and event date, checked against the reported make and model; the registry's history has gaps, so older matches can be wrong. Compare the reported make and model. For accidents, the NTSB's own investigation is the fuller record.
